According to records, drilling occurred 1/23/1997-1/26/1997. Well tag present, legible. Access road stabilized, gated.Department observed areas adjacent to brine tank characterized by wet, muddy soils and a lack of vegetative growth which may be evidence of a prior release and/or discharge. One potentially-impacted area appears to originate near tank valve and is approximately 3-4’ x 5-6’ in size. There is another potentially-impacted area behind the brine tank that appears to be approximately 7’x4’ in size. Soil samples were collected up-gradient from the brine tank, within the potentially-impacted area near the tank valve, and within the potentially-impacted area behind the brine tank.This inspection is currently pending upon the Operator’s response and/or receipt and review of the sample results.
2465148 Primary Facility 2016-03-09 Administrative/File Review Outstanding Violations - No Viols Req'd Admin inspection in response to violations issued under inspection ID 2455369 for failure to properly control and dispose of residual fluids.

At 1242, Operator representative Doug Forbes notified the Department via phone that the contaminated soils have been excavated and the site has been seeded/mulched. The contaminated soils are contained within 4 drums. Mr. Forbes was unable to quantify the fluids released at this time. The Department recommends that the Operator ensure the spill is and/or has been remediated in accordance with the Department’s Spill Policy. A follow-up inspection will be conducted to ensure the site is properly remediated and restored. At this time, the Department is requesting that the Operator submit all disposal records/manifests to the Department upon final disposition of all wastes.

2471007 Primary Facility 2016-03-31 Administrative/File Review Outstanding Violations - No Viols Req'd Admin inspection in response to violations issued under inspection ID 2455369 for failure to properly control and dispose of residual fluids.

The requested NOV written response was received by the Department on 3/31/2016 and has not yet been reviewed. Violations remain outstanding under inspection ID 2455369 (enf ID 340619).

2473258 Primary Facility 2016-04-05 Administrative/File Review Outstanding Violations - No Viols Req'd Admin inspection in response to violations issued under inspection ID 2455369 (enf ID 340619) for failure to properly control and dispose of residual fluids.

Results of soil samples collected by Department (inspection record 2455369) received from BOL. The results appear to be indicative of a production water release as both potentially-impacted soil sample results reveal that the following parameters were elevated when compared to the background soil sample: barium, calcium, chloride, iron, magnesium, potassium, sodium, and strontium. The Operator previously identified these areas of potentially-impacted soils as areas where releases did occur and/or may have possibly occurred (inspection ID 2455369). Violations remain outstanding under inspection ID 2455369 (enf ID 340619).

2477675 Primary Facility 2016-04-12 Follow-up Inspection Outstanding Violations - No Viols Req'd On-site FUI conducted in response to violations issued under inspection ID 2455369 (enf ID 340619).Opr well-tender George Llewellyn on-site during inspt.At time of inspt, areas of previous releases appear to have been excavated, backfilled, seeded, and mulched with minimal to no vegetative growth observed.Excavated materials not on-site.Dept previously requested all disposal records/manifests to the Dept upon final disposition of all wastes.Not yet received.WT present, legible. 2480542 Primary Facility 2016-04-28 Compliance Evaluation No Violations Noted Admin inspection in response to violations issued under inspection ID 2455369 (enf ID 340619) for failure to properly control and dispose of residual fluids.

At 0846, the Department called for Operator representative Dickenson Forbes via phone and left a message. At 0946, Mr. Forbes returned the Department’s phone call and left a message. At 1037, the Department reached Mr. Forbes via phone. After reviewing the NOV response, an estimated quantity of 2 gallons released was provided for the discharge near the brine tank valve. No quantity was provided for the release on the opposite side of the tank; therefore the Department requested a quantity be provided. Mr. Forbes provided an estimated quantity released of no more than 2 gallons, for a total of 4 gallons released. Hence, as documented in inspection ID 2477675, it appears as if the Operator is remediating the release in accordance with the Spill Policy.
