2385142 Primary Facility 2015-06-26 Administrative/File Review In Compliance with Policy Admin inspt in response to record 2383851 for reportable release of 1 bbl brine water.Upon receipt of spill report, Dept contacted Opr rep J. Canose via phone on 6/26/15 at 0735.Mr. Canose stated release was result of slow drip from crack in bottom of fiberglass BT.Release first identified when small areas of dead veg were observed around tank by well tender.Tank was drained, removed from its graveled location, and a conductivity meter utilized to delineate release.Measurement calcs indicate that at least 1 bbl of brine water was lost.Mr. Canose is in process of verifying this quantity at time of this phone call.Roll-off box(es) to be delivered to location today.Excavation will be dependent on weather as heavy rains are forecasted.Dept recom’d to Opr that impacted area is covered w/tarp and/or plastic to prevent any further migration of contaminants should excavation be postponed due to weather conditions.
2385222 Primary Facility 2015-06-26 Incident- Response to Accident or Event Violation(s) Noted On-site inspt conducted in response to record 2383851 for 6/25 reportable release of 1 bbl brine water mixture from BT.At time of inspt, excavation of impacted materials not yet commenced.Roll-off box(es) not yet on-site.BT disconnected,re-located to edge of site. Area of stressed vegetation extends approx 10-15 feet down-gradient of former BT location.WT present, legible.According to records, drilling occurred 2/23/2001-2/27/2001.Dept recommends Opr cont' to monitor/maintain AR and well site as necessary and install temp E&S BMPs as appropriate prior to commencement of earth disturbance activities in order to prevent any add'l pollution events from occurring.A follow-up inspection will be conducted in the future to ensure site is properly remediated and restored.
2411900 Primary Facility 2015-09-22 Compliance Evaluation No Violations Noted On-site CEI conducted in response to viols issued under inspection 2385222 (enf ID 325173) for 6/25 reportable release of 1 bbl brine.At time of inspt, all visibly impacted materials appear excavated.Area of excavation not yet backfilled, surrounded by orange security fence.Roll-off box(es) not on-site.BT appears re-equipped to well and appears sound. WT present, legible.Dept recommends Opr cont’ to monitor/ maintain site to ensure >70% adequate veg is attained for permanent stabilization of site upon backfilling.In accordance w/Spill Policy, 15 day written response received by Dept on 7/13, notifying Dept that Opr will be entering Act 2 for remediation of release.As Opr has identified intent for remediation and excavation of contaminated soils has commenced, viols issued under inspection 2385222 (enf ID 325173) corrected.A FUI is anticipated to be conducted to ensure site is properly restored per current regs.Viols may be forthcoming pending future inspts monitoring site restoration and permanent stabilization.

Violations were issued for this reportable release under inspection record 2385222 (enf ID 325173) and subsequently corrected under inspection record 2411900.

At time of inspection, well drilled and appears equipped for production. According to records, drilling occurred 2/23/2001- 2/27/2001. Well tag present, legible. Brine tank appears sound. Area of excavation backfilled and adequately vegetated for permanent stabilization. Temporary BMPs still installed down-gradient of previous area of excavation. As the site has achieved permanent stabilization, the Department recommends the Operator properly remove and dispose of the temporary BMPs in accordance with current regulations. Located adjacent to ag field. Access road appears to be shared, developing ruts. The Department recommends the Operator continue to monitor and maintain the access road as necessary in order to prevent any
