2607933 Primary Facility 2017-06-21 Administrative/File Review In Compliance with Policy Admin inspection conducted as follow-up to inspection record 2606154. At 1002, Department spoke to Operator representative Michael Bucheit via phone.

Per Mr. Bucheit: The staining on the gravel observed by the Department is residue/emissions from the exhaust system of the pump jack. The exhaust system should not be laying on the ground as observed by the Department. The exhaust system will be reconfigured and a containment unit installed around the system exit in order to hold any liquid/solid pollutants that may be emitted in the future.

Mr. Bucheit is unable to identify the constituent(s) released at this time. All visibly impacted materials will be promptly removed in accordance with the Department's Spill Policy. Mr. Bucheit will continue to follow-up with the Department.

The Department is requesting that the Operator submit the waste records/manifests to the attention of Samantha Foulk at the address provided upon the final disposition of wastes.

2734741 Primary Facility 2018-05-03 Complaint Inspection Violation(s) Noted Well appears equipped for production. No brine tank on site. Pump jack on site.

Department observed an active release of what appears to be brine/oil (photo #1). This fluid is leaking from pipe on the south west corner of the site and creating a wet area approximately 2.5'X10" (photo #2). Department contacted operator rep Doug Wadsworth by phone from site. Mr. Wadsworth and well tender responded immediately. Well tender stated he visited site on Monday (4-30-18) and did not observe a release. Well tender also stated well is currently shut in. Department recommends operator follow up with Department regarding site remediation plan/schedule. A violation is being issued fro failure to collect brine or other fluids. A follow up inspection will be conducted at a later date.

2738329 Primary Facility 2018-06-06 Administrative/File Review Outstanding Violations - Viols Req'd ADMIN inspection to inspection record #2734741. Operator rep Michael Bucheit contacted Department by phone. Mr. Bucheit requested clarification on what is required in the operator response to the NOV. Department addressed Mr. Bucheit's questions and concerns.

Violations issued on 5-3-18 remain outstanding. A follow up inspection will be conducted at a later date.

Department contacted operator rep Michael Bucheit by phone from site for an update. Mr. Bucheit stated no impacted material has been removed from the site, they are waiting on sample results before excavating. Department requests disposal,manifests be provided when they are available. Violation issued on 5-3-28 remains outstanding.
