Well head consisted of 8¿ pipe to 4¿ to 2¿ production pipe with shut off valves. Production equipment consisted of separator; totalize meter (8820 MCF) and 100 bbl brine tank (with < 10 bbl fluid). At the time of inspection, no IW and E&S issues at the site. No odor of gas leakage at well head. Missing well tag appears to be painted over during painting of wellhead. DEP request for a new well tag.

2424320 Primary Facility 2015-11-10 Administrative/File Review In Compliance with Policy Admin inspt conducted in response to spill reported under inspt ID 2424284 for reportable release of 35 gal brine.At 1400, Dept contacted Opr rep J.Canose via phone.According to Mr. Canose, release resulted when tank was being prepared to be emptied by water truck.An attempt was made to remove plug on valve w/a wrench;however,threads were corroded,which caused brine to be released from tank before hose could be connected for proper emptying.Per Mr. Canose, hose was immediately placed under draining fluids until tank emptied.Fluids traveled onto ground and into nearby tire rut.Brine was immediately vac’d up.Mr. Canose is estimating release of 35 gal brine,will submit spill calcs to Dept.According to Mr. Canose, excavation of all visibly impacted materials has commenced,two roll-off boxes en route to site.No waterways reported to be impacted.Opr appears to be ICP at this time.A FUI will be conducted to assess any environmental impact and/or issue any associated violations as necessary.
2424452 Primary Facility 2015-11-12 Incident- Response to Accident or Event Violation(s) Noted On-site inspt conducted in response to admin inspt 2424284 for 11/10 reportable release of 35 gal brine from production tank.At time of inspt, all visibly impacted materials appear excavated in accordance with the Dept’s Spill Policy.Temp BMPs installed down-gradient of excavated areas.Two covered roll-off boxes on-site.BT disconnected and moved to edge of well site.WT present, legible.According to records, drilling occurred 7/22/04-7/24/04.Shared AR stable.Dept recommends Opr ensure all disturbed areas on well site are temp stabilized w/in 4 days of cessation of earth disturbance activities per current regs.A FUI will be conducted to ensure site is properly remediated and restored.Viols are being issued for this reportable release of production fluids.At this time, the Dept is requesting the Opr submit all disposal records/manifests upon final disposition of wastes to the Dept at the provided address.

2442690 Primary Facility 2016-01-21 Compliance Evaluation No Violations Noted On-site CEI conducted in response to violations issued under inspection ID 2424452 (enf ID 331567) for the 11/10 reportable release of 35 gal brine from production tank.At time of inspection, site appears to have been backfilled, however due to snow-cover, Department unable to adequately verify completion of backfilling. During inspection, Department contacted Operator representative Jason Canose via phone. Mr. Canose verbally confirmed the completion of backfilling at this site.Temporary BMPs remain installed. Two covered roll-off boxes on-site no longer remain on-site.A follow-up inspection is anticipated to be conducted in the future to ensure site is properly restored as per current regulations. Violations may be forthcoming pending future inspections monitoring site remediation and permanent stabilization.Violations issued under inspection ID 2424452 (enf ID 331567) have been corrected.
