According to records, drilling occurred 4/10/2007-4/13/2007. Brine tank appears sound. Fixation of well tag compromised. Department recommends Operator replace well tag to ensure well tag is installed on the well in a legible, visible, and permanent manner as per current regulations.

Well site permanently stabilized with >70% uniform and adequate vegetative growth. No accelerated erosion and/or sedimentation issues observed in relation to well site. Minor potholes in access road. Department recommends Operator continue to monitor and maintain access road as necessary in order to prevent any potential pollution events from occurring.
