1841569 Primary Facility 2009-10-27 Routine/Complete Inspection No Violations Noted At the time of inspection, the well was drilled not yet in production. The site was properly signed with E&S plan onsite available for review. No accelerated erosion or sedimentation issues were observed in relation to access road or well site. Fluids were removed from pits, but the pit liners were improperly secured, as small portions of the upper liner had become unsecured and fallen downward, into the pit. Although freeboard was not exceeded this could cause a potential pollution event, it is recommended that his be addressed promptly to avoid any such violation.

No accelerated E&S issues were observed. Access road was stable. Site was regraded and rocked, but it is recommended that operator reseed site as 70% successful vegetative growth was not yet observed. A future inspection will be conducted to ensure site is reseeded and mulched to optimal/uniform growth to ensure permanent stabilization.
