Filter fence located topographically down gradient of site pad. Conductor pipe set at 42.28 ft. 9 5/8¿ pipe set at 412.30 ft with 150 sks class A with 9 bbl return. Water indicated at 120 ft. and 560 ft. No gas indicated nor voids on the boring logs. Stratigraphy consisted of shale, sandstone, limestone, and coal seams.

1842219 Primary Facility 2009-11-02 Routine/Complete Inspection No Violations Noted At the time of inspection, the site was properly signed and E&S plan was available for review. The rig was setting up in preparation for drilling. Temporary stream crossing was correctly constructed. Filter fabric fence had been properly installed and functioning along with a grub/brush filter. No accelerated E&S issues were observed in relation to well site or access road, although operator should continue to monitor access road for erosion and sedimentation due to slope of roadway. Pits were properly constructed, but not yet lined, reminded operator to properly secure liner when placing in pit. All disturbed areas were temporarily seeded.
1928294 Primary Facility 2010-11-18 Site Restoration No Violations Noted During this inspection, well properly tagged and in production. Site restored with 70% vegetative cover. It is recommended that operator reseed cut slope in spring to ensure uniform coverage. No accelerated erosion or sedimentation issues were noted in relation to well site or access road. No IW concerns noted. Brine tank stable. Operator may remove filter fence as 70% vegetative, permanent stabilization has been reached. Access road stable. Minor stream crossing(GP-7 approved) properly installed and functioning; appers riprap has been add to stream banks; therefore hay bale reinforced filter fence may be removed if permanent stabilization to stream bank/bottom is maintained along crossing
