Access road stable. Multi well pad. Well drilled and equipped for production. Well spudded 9/11/09, drilling completed 10/28/09. Ten brine tanks located onsite which are surrounded by rock dike. Tanks appear structurally sound, although noted hose connected to main discharge line (manifold) of all tanks, it is recommended that operator remove hose when not actively emptying tanks in order to prevent a possible pollution event or even acts of vandalism. Bmp's installed according to plan and it appears that site has been stabilized with vegetative matting, although limited growth <70% uniform coverage observed. No accelerated erosion or sedimentation issues noted on pad or access road, department observed several sections of super/reinfoced silt fence that are structurally impaired-- therefore operaor should maintain.

Five brine tanks located in lined secondary containment all appear sound. Access road and well pad stabilized. Well site permanently stabilized with vegetative growth. No accelerated erosion and/or sedimentation issues observed during this inspection. No violations noted at this time. A release of 5 gallons glycol was reported at this well site (inspection ID 2553738) on 1/9/17. Please refer to inspection ID 2553758 for the inspection conducted in response to this reported release.

There are 5 wells drilled and equipped for production. The well pad is permanently stabilized. The Emergency Response Plan and PPC Plan are located at the pad entrance. A site ID sign is posted and a well tag is affixed to the well head. There are no violations noted for this inspection.
