Bill Kolovich Construction Superintendent (per his request) was onsite for a review of minor additions made to the E&S plan. The site was properly signed. The well pad was currently in the early construction phase, clearing of land. I noted and approved the addition of curlex ditches and culvert pipes on the E&S plan. I did not observe any accelerated erosion or sedimentation during this construction phase and it appeared that all installed BMP¿s were functioning properly.
1847592 Primary Facility 2009-12-04 Drilling/Alteration No Violations Noted At the time of inspection, no E&S or IW issues at the site. Well sign and E&S plan located at the entrance of site. Pit aligned with 20 mil plastic. No freeboard issues. Conductor pipe set at 28 ft ¿ 24 ¿pipe used 50 sks cement. Current depth at 120¿ Second stringer 11 ¾¿ pipe will be set at 395 ft (deep water well). Permit and sedimentary log present. Water not recorded. Sedimentary consisted of ss/sh alternation and coal seam. No voids or methane detected. Filter fence set around the site. Permit set for 9000 ft well to Marcellus. During the time of inspection, frac pit was being filled from POTW fire hydrant.

Pit aligned with 20 mil plastic. No freeboard issues. Filter fence set around the site. Upper section completed to 1900 ft. Two Brine tanks delivered to site.

Pit aligned with 20 mil plastic. No freeboard issues. Filter fence set around the site. Drillers pit closed. Wellhead located in secured fence.
1914161 Primary Facility 2010-09-07 Routine/Complete Inspection No Violations Noted At time of this inspection, Chad Eisenman of Atlas also onsite. Site properly signed with E&S plan available for review. Well is drilled not yet set for production. Noted the proper installation of all bmp¿s according to plan. No accelerated erosion or sedimentation issues were noted, although site has not yet been restored, Atlas currently is in the process of applying for a restoration extension. Well completed on 02/19/10, restoration deadline 11/19/10. During this inspection, one empty, lined pit onsite. Site/Well is currently being monitored for hydrogen sulfide, as relatively high levels were detected at a nearby well. Security fence was placed around pits and well head. A follow up inspection will be conducted to evaluate final (permanent) restoration activities, until that time operator should continue to maintain all bmp¿s onsite and along access road.
1958250 Primary Facility 2011-03-08 Routine/Complete Inspection No Violations Noted well drilled not yet in production. Well completed 2/19/10. Site re-graded (pits removed) seeded & mulched with some growth, although it is recommended operator re-seed & mulch to ensure all areas of site exhibit a minimum, uniform vegetative cover of at least 70% across the site. Access road and well side stable. No accelerated erosion or sedimentation issues noted. Two brine tanks onsite appear stable. All temporary bmp¿s installed & functioning. Spoke with Jeff Zarella Atlas, via telephone on 3/9/11, whom stated he thought an extension was filed for restoration of this site, but never received a response either way. Dept will further look into the matter to determine if indeed an extension was filed and or approved or denied. At this time no violations are noted, but operator shall ensure optimal, uniform growth is achieved and site is permanently restored as a follow up inspection will be conducted in the near future.
