Access road was stable and sufficiently rocked. Below grade pit on site that sustained 2ft of lined freeboard. All bmps were installed and functioning as noted in E&S plan. No accelerated erosion or sedimentation issues observed in relation to well site or access road. It is recommended that operator temporary seed soil pile or line with filter fence to avoid any possible accelerated E&S issues. Operator should also continue to closely monitor all bmp¿s until final restoration activities have been completed.
1894669 Primary Facility 2010-06-23 Drilling/Alteration No Violations Noted At the time of inspection, no E&S or IW issues at the site. Pit aligned with 30 mil plastic and no freeboard issues. Northeast set conductor pipe (13 3/8¿) 42 ` (sanded in) and 9 5/8¿ pipe set to 421 ft with basket at 84¿ and 155 sks with 9 bbl return. And 7 ¿pipe to 1308 ft with 105 sks Appellate and 75 sks Class A with 10 bbls return. Current depth was 3510 ft. Water detected at 250 ft. Spud called on 6/13/10. Well permit and cement receipts present along with well log. Stratigraphy consisted of sandstone, shale and coal. Well sign and E&S plan located at the entrance way of site. Down gradient filter fence present.
1915949 Primary Facility 2010-09-22 Routine/Complete Inspection No Violations Noted At the time of inspection, Site was properly signed with E&S plan. Well drilled and in production. Access road was stable and sufficiently rocked. All bmps were installed and functioning as noted in E&S plan. No accelerated erosion or sedimentation issues observed in relation to well site or access road. No IW issues observed. Operator should also continue to closely monitor all bmp¿s until final restoration activities have been completed. Pits have been removed, but site has not yet been re-graded or seeded /mulched. Accompanied by WQS Dana Burk during inspection.
1931747 Primary Facility 2010-12-02 Routine/Complete Inspection No Violations Noted At the time of inspection, no E&S or IW issues at the site. Well completed with 8¿ x 4¿ pipe and reduced to 2¿ production line. Line connected to data logger, separator, heater and 100 bbl brine tank (<10 bbl fluid). Data logger showed well turned on briefly. No signs of leakage around well head. Well tag present.
1955486 Primary Facility 2011-03-07 Routine/Complete Inspection No Violations Noted site Id sign and Es plan present. Access road stable. Well completed on 06/24/10, 9 month restoration deadline is 03/24/11. As of this inspection, site is re-graded, but growth not yet observed; therefore, site restoration is not complete. (If operator is unable to meet 9 month deadline due to adverse circumstances as outlined in 78.65 (2) , operator may file for a restoration extension, , but approval is at the discretion of the dept provided amply evidence is presented to support claim for extension.) Appears operator has seeded and mulched site. Temporary bmp¿s (filter fence and brush/grub line) are installed, but operator should continue to monitor and maintain all bmp¿s until final permanent restoration is achieved. Observed minor ES (ponding of water) concerns near well head/production equipment possibly due to uneven grade, but no accelerated (offsite) erosion or sedimentation issues were noted. Faint gas odor detected near production equipment. Follow up inspection will be conducted to ensure permanent/final restoration is achieved.
2295828 Primary Facility 2014-05-07 Routine/Complete Inspection No Violations Noted At the time of inspection, no E&S or IW issues at the site. Gas well was in production. Annulus was open
