1943442 Primary Facility 2011-01-11 Routine/Complete Inspection Violation(s) Noted Spudded 12/28/10. Drilling complete. HQ watershed. Site id sign, E&S plan ok. Access road, and site snow covered, but appear ok. Compliance assistance inspection on 1/5/11 used to address issues with pit and disposal of cuttings in relation to SHWT. Also spoke to operator representative Joanne about this. Inspection revealed the following deficiencies: 1) Drill pit constructed less than 20 inches from SHWT. According to E&S plan depth to SHWT 6 to 12 inches. Pit was already 8 to 10 feet below surface due to cut. 2) Improper encapsulation of drill cuttings. Pit was not minimum of 20 inches from SHWT so could not encapsulate. These were discussed with operator before violations commited. Operator said no actions would be taken until soil examined, then called few days later to inform that pit was encapsulated and no soil evaluation done. Drill cuttings, liner, and contaminated soil must be removed from site of encapsulation, taken to an approved disposal facility, and all disposal recipts/manifests submitted to the Department for verification.
2227486 Primary Facility 2013-12-04 Follow-up Inspection Outstanding Violations - No Viols Req'd FUI in response to viols issued under inspt record 1943442 (enf record 266930) for failure to construct pit so that bottom of pit is greater than 20 in from SHGWT and for failure to properly encapsulate drill cuttings.At time of inspt, well drilled and equipped for production.BT sound. Shared AR stable.Site uniformly and adequately vegetated, permanently stabilized.Legibility of well label compromised; Dept recommends Opr affix WT to well in a visible, permanent, and legible manner as per current regs.To date, Dept has not received evidence from Opr documenting the SHGWT to be greater than 20 in from bottom of pit as stated in written response received on 1/31/2011.As per inspt record 1943419, Opr stated a soil scientist would determine the depth of the SHGWT prior to pit encapsulation.No soil survey completed.Outstanding viols exist under PA 25 CODE 78.56, 78.57, 91.35, 78.62.No new viols noted at this time.Spoke to Operator representative, Joanne via phone at 1025. Department requested information. Additional comments on paper copy.
