Bmp's installed according to plan and it appears that site has been stabilized with vegetative matting, although limited growth <70% uniform coverage observed. Final restoration could not be determined , well site is partially snow covered. A follow up will be conducted in future to evaluate final permanent site restoration.Ten brine tanks located onsite,surrounded by rock dike. Tanks appear structurally sound, noted hose connected to main discharge line (manifold) of all tanks, it is recommended that operator remove hose when not actively emptying tanks in order to prevent a possible pollution event or even acts of vandalism. No accelerated ES issues on pad/access road, but dept observed impaired filter fence which should be mainained.

AT 1646 ON 1/9/17, A RELEASE OF 5 GALLONS GLYCOL ONTO THE PRODUCTION PAD WAS REPORTED TO FIRST CHOICE TAS BY OPERATOR REPRESENTATIVE COREY GILES. NO WATERWAYS REPORTED TO BE IMPACTED. RELEASE IS CURRENTLY BEING CLEANED UP.
2553751 Primary Facility 2017-01-10 Administrative/File Review In Compliance with Policy Admin inspection conducted in response to inspection ID 2553738 for the reported release of 5 gallons glycol.

Upon receipt of spill notification, Department contacted Operator representative Corey Giles via phone at 0818. According to Mr. Giles: A total release of 20 gallons glycol occurred – 15 gallons remained in secondary containment and 5 gallons was released outside of secondary containment. The spill resulted from a malfunctioning pressure relief valve, which caused the glycol to be released as a mist. The glycol did not come into contact with the ground as the pad was snow-covered and the glycol remained on the surface of the snow. Per Mr. Giles, the release was discovered yesterday (1/9/17) at 0215 by the well-tender while it was actively occurring. Mr. Giles stated that the visibly impacted snow was immediately removed and placed into drums for proper disposal. The Department is requesting that the Operator submit all disposal records/manifests for any wastes removed from the site in response to

2553758 Primary Facility 2017-01-11 Administrative/File Review No Violations Noted On-site inspection conducted in response to inspection ID 2553738 for the reported release of 5 gallons glycol.

At the time of this inspection, two Operator representatives currently on-site performing maintenance activities. According to the on-site Operator representatives: The glycol release occurred due to a “bad gasket”. Approximately 15 gallons of glycol dripped into secondary containment, while approximately 5 gallons of glycol misted out onto the snow outside of secondary containment. The impacted materials were removed and placed into a drum, which is currently on-site awaiting proper disposal. The well site was not snow-covered during this inspection. At this time, the Department did not observe any adverse environmental impact that may have resulted from this reported release of glycol. SDS for glycol available on-site. Site ID sign installed. There are 5 wells drilled and equipped for production. The well pad is permanently stabilized. The Emergency Response Plan and PPC Plan are located at the pad entrance. A site ID sign is posted and a well tag is affixed to the well head. There are no violations noted for this inspection.
