No accelerated erosion or sedimentation issues noted, although observed formation of algae in drainage ditches possibly due to stagnant water. Recommend operator continue to monitor and maintain (as necessary) all bmp's to ensure that are properly functioning in order to prevent a possible pollution event. No violations noted during this inspection

Multi well pad. Site is surrounding by wire fence in order to prevent third party access. Drilling completed 6/18/11. Pad adequately rocked. Wells drilled, not yet fraced nor equipped for production. Site is temporarily stabilized. No ES concerns noted. Operator has plans to permanently restore well site March 2013, a letter has been submitted discussing restoration efforts and timeline to Alan Eichler Program Manager OG SWRO. Hence prior arrangements for restoration to PCSM standards have been made no violations are currently noted; although operator should continue to closely monitor and maintain site and access road until final permanent restoration is achieved as outlined in memo. Chevron EHS personnel, Curtis Small, OGI Rick Hoover, and Environmental Trainee Samantha Foulk on site during inspection.

2473113 Primary Facility 2016-03-31 Routine/Partial Inspection In Compliance with Policy On-site inspection as requested by Operator representative Curtis Small to discuss requirements pursuant to well plugging.The Operator is planning to potentially plug the two wells (063-37037, 063-37038) located on this pad. Instead of restoring the site to approximate original contours in accordance with current regulations, the landowner would like the gravel on the pad removed and replaced with topsoil. The Department recommends the landowner obtain an NPDES permit from the Indiana County Conservation District for this location upon plugging of the wells. The Department recommends that the Operator provide a copy of this permit to the Department to document the transfer of responsibility for this location in lieu of restoring the site to approximate original contours.
