2026871 Primary Facility 2011-12-12 Drilling/Alteration No Violations Noted PC Rig #6 was contracted by Penn View (Jason Kline ¿ tool pusher). Well sign and E&S plan located at the access road. Filter fence was currently located topographically down gradient of site pad. No freeboard issues. During site inspection, no E&S or IW issues at the site. Water was recorded at 50 feet. Intermediate pipe (7¿) was set at 1,095feet and was cemented with 200 sks of Class A. Plug was down at 2:15 am on 12/12/11with 15 bbls return. 80 sks of cement was grouted to the 9 5/8¿ pipe to bring the cement to surface. PC was currently drilling at 2,820 feet for the production string. The anticipated depth set at 4,950 feet.
2026875 Primary Facility 2011-12-08 Drilling/Alteration No Violations Noted PC Rig #6 was contracted by Penn View (Jason Kline ¿ tool pusher). Well sign and E&S plan located at the access road. Filter fence is currently located topographically down gradient of site pad. No freeboard issues. During site inspection, no E&S or IW issues at the site. Conductor pipe (13 3/8¿) was set at 22¿ (sanded in place). Water protection (9 5/8¿) was set at 211 ft. and cemented with 80 sks of Class A. Plug was down at 8:44 pm on 12/7/11with no return. Water was recorded at 50 feet. Phillips was setting up to drill the 7¿ pipe to 1095 feet.
2028569 Primary Facility 2011-12-13 Routine/Complete Inspection No Violations Noted At the time of inspection, well sign ES plan present. Well drilled not yet equipped for production. Spud date 12/6/11. Rig onsite rigging down. One below grade pit onsite, pit contents reaching 2ft freeboard, recommend operator remove material as soon as possible in order to prevent possible pollution event.Operator plans to dispose of pit (non-liquid) contents onsite according to regulation. No security fence located around pit at time of this inspection. Bmp's appear to be installed and functioning as outlined in Es plan. Access road ok, entrance adequately rocked.
