2073722 Primary Facility 2012-06-05 Routine/Complete Inspection No Violations Noted During this inspection, XTO rep Tom Baun and First Class Energy Rep( Construction Contractor) present. Site ID sign, ES plan, ESCGP-1 located onsite. Well not yet drilled. Minor changes have been noted on ES plan, the addition of a rock lined culvert along access road to well site and removal of culvert (per Penn Dot due to drainage on SR 410). Previously disturbed areas covered with matting and seeded/mulched with observed growth not quit 70% uniform coverage. Pad adequately rocked. No accelerated E&S issues on well pad. Secondary containment located onsite appears sound. XTO reps and myself examined access road leading to a impoundment, not yet constructed. A culvert was installed and a sediment basin, it appears that due to grade and recent heavy rains the sediment basin could not handle flow; therefore, XTO would like to install a water bar leading to culvert located down gradient near well pad entrance. Dept ok'ed no increase in acreage or disturbed area-

2108425 Primary Facility 2012-10-15 Routine/Complete Inspection No Violations Noted At the time of this inspection wells actively being fraced. Bmp's installed and maintained, noaccelerated erosion or sedimentation concerns noted on access road or well site. Hence well site is located with HQ waters, department reiterated to operator that deflectors are not considered ABACT bmp's and in order to sustain sufficient sediment removal an additoinal ABACT bmp is reccomended, department referred operator to currentl ES manual for list of acceptable ABACT bmp's. Operator agreed to install water delfector in access road, as well as add additional compost sox in order to increase the effiencety of sediment removal. Based on this inspection, if operator installs/maintains additonal bmp's as outlined in ES manual and revised plans department shall ok minor changes as noted.
2128933 Primary Facility 2013-01-08 Routine/Complete Inspection No Violations Noted well site Id sign, & ESCGP-1, ES plan on site. Site located within HQ Waters. Well not yet drilled-permit issued 1/4/12 , permit expiry 1/4/13. As of today, permit has expired and no valid drilling permit located onsite. Therefore, unless a (re)new permit is obtained site shall be restored within nine months of TD date (7/8/12) of last well drilled, 1H. Access road entrance adequately rocked, although due to recent thaw and continual traffic on/off site, due to pipeline installation, recommend that operator closely monitor and maintain access roadway as needed to ensure that mud does not exit the well site onto SR 210. Well not yet drilled. Site snow covered; therefore, unable to adequately assess temporary stabilization measures and temporary bmp functioning capacity. Although no accelerated erosion or sedimentation issues observed. Department recommends that operator install a temp bmp beneath soil pile located on back side of well pad in order to prevent a potential pollution event as a result of recent thaw. No viols noted.
