Since the most recent inspection of this well on 1/7/2016 (Inspection Record # 2439420), the site has been restored, and the Act 2 report regarding the leak in the flowback impoundment has been approved. The ESCGP-1 associated with this well is ESX11-065-0027. The well has been drilled and frac'ed, and is in production. The correct DEP permit number has been posted on the wellhead. At the time of inspection, vegetation has become established and all E&S controls remain in place.

An undrilled permit had been issued for this site; the Gustafson Unit # 7H (065-27001). This un-drilled permit expired 11/21/2016. The ESCGP was renewed as an ESCGP-2 and now expires on 12/19/2021. Temporary E&S controls can be removed since vegetation has achieved greater than 70% cover.
