The well pad remains at the originally permitted size and PCSM was implemented for the entire well pad area as permitted by the Lycoming CCD. The well pad is being used as a storage yard for various materials. Please remove the materials/equipment or provide the landowners consent for storage of materials onsite under § 3216 (c) OGA. At some time since the original pad was constructed, the access road was re-routed to accommodate the Barto Compressor Station. The Barto Compressor Station and the re-route of a portion of the access road to your site were covered under ESCGP-1 #4108803 issued to Chief Gathering and currently owned by Regency Energy (Energy Transfer Partners). The plans for the Barto 1-6H well pad do not reflect the current conditions onsite and the permit has now expired. The Department requests you re-permit this well pad from the LOD of the adjacent ESCGP-1# 4108803 to accommodate the well site
2658495 Primary Facility 2017-10-25 Routine/Complete Inspection No Violations Noted The PCSM/Well Site Restoration plans were recently implemented and stabilized with hydro-seed. The footprint of the well pad surface has also been reduced. The 2-part outlet structure on the infiltration basin at the entrance to the site is currently not sealed. This condition currently allows water to discharge several inches below the design capacity. In addition, implementation of the two infiltration basins is not functioning as designed. Currently, water is discharging between the two infiltration features. This concentrated-flow is causing erosion and renders the structures ineffective. Erosion and sedimentation was not noted outside the perimeter E & S controls. I recommend addressing the stabilization, erosion, and PCSM noted above. No violations noted.
2694336 Primary Facility 2018-02-14 Routine/Complete Inspection No Violations Noted Weather was 30° and cloudy at time of inspection. Well has a digital gauge attached to the swab valve and also to the 5 1/2 casing valve. The digital gauge attached to the swab valve read 240 psi and the 5 1/2 casing gauge read 418 psi at time of inspection. There were 2 annulus vents observed during the inspection, but it was unable to be determined where each vent originated from. 0% combustible gas was detected in any location at time of inspection. There was an illegible well tag laying on the ground by the well however no legible well tag with the API number was attached to the well at time of inspection. No violations were noted at time of inspection.
