The site was being reclaimed and returned to approximate original contours. An infiltration trench and a vegetative filter strip have been left in place while the main earthmoving is being completed. 2501112 Primary Facility 2016-07-12 Complaint Inspection No Violations Noted Complaint inspection (CID 320890); 7/12/2016.

Dan Harvey (DEP) and I met with the complainant, Mike Hahn (XTO), and Mike Vale (XTO consultant). At the time of the inspection, the well site was reclaimed to approximate original contours and seeded and stabilized. The E&S module from 2009 does not include any relevant details or a plan of the final reclamation (after wells are plugged) of the well site.

The complainant had 4 major concerns about the well site including the vegetation, topsoil, existing surface rock/piles, and the restoration of a pre-existing drainage swale. XTO agreed to install the 10’ wide drainage swale as designed in the plans attached. These plans were provided to the complainant for approval but were not previously submitted to the Department. We all agree that the designed swale will adequately restore the site to its approximate pre-construction condition and function. XTO also agreed to re-grade a portion of the rock pile/rock outcrop (41-19-40.90N, -76-43-07.66W) in the field

2502504 Primary Facility 2016-07-18 Routine/Complete Inspection No Violations Noted Complaint inspection (CID 320890); 7/18/2016.

I arrived onsite at 10:45 and met with Mike Hahn (XTO) and the construction contractor. The swale across the field had been installed and the operator was working on smoothing the area around the rock pile/ rock outcrop. The swale appears to be constructed as depicted in the plans attached to my last inspection (7/12/2016) of the site. No Violations Noted.

2537429 Primary Facility 2016-11-04 Routine/Complete Inspection No Violations Noted Well Site Restoration/NOT Inspection; 11/4/2016.

At the time of the inspection, the former well pad area was had been mowed and the warm season grasses turned brown. The site has been restored to approximate original conditions and is permanently stabilized. I recommend the Well Site Restoration Report and Notice of Termination be approved for this site. No Violations Noted.
