At the time of the inspection, the Farnsworth Unit 1H (081-20769) was shut-in after completions. Both the Well Record and Completion Report were submitted. No pipeline is connected to the well pad and the well has never been in production. The well pad remains at its original size as per the minor modification approved on 6/20/2012 but has not been restored as depicted in the PCSM plans. No other wells were ever permitted for the site and the ESCGP expires on 10/16/2016.

The Department considers a well site to be restored under Section 3216(c) of the 2012 Oil and Gas Act when the oil or gas owners or operators meet the following:

1. All permanent post construction stormwater control features as identified in the Post Construction Stormwater Management/Site Restoration Plan are in place.

2. Remaining impervious areas are minimized. See Section 102.8 (b)(4). Impervious areas include, but are not limited to, areas where the soil has been compacted, areas where the soil ha

2841519 Primary Facility 2019-01-28 Routine/Complete Inspection No Violations Noted Department representative Jared Jacobini conducted a routine site inspection at Farnsworth Unit 1H (081-20769) on January 28, 2019. The site sign, emergency response plan, and weekly inspection log were onsite for review.

The well site has one shut-in well (1H). No active permits for additional wells are permitted with DEP. The 1H well was finished drilling on 6/24/2012. Based on the drill date, this site has not been restored to Site Restoration Plan Specifications, and PCSM's have not been implemented. The well remains in active status, but it clearly is not being produced due to the lack of production equipment. The well will be referred to the Oil and Gas inspector for further review.

Recommended Site options:

Implement PCSM's and apply for a 2-year restoration extension if the site and tank pad will be utilized.

Or

Restore site per site restoration plan described in the ESCGP if the site will not be utilized.

Or

Consider plugging and full restoration if the site will not be utilized.
