2812329 Primary Facility 2018-12-06 Incident- Response to Accident or Event No Violations Noted On site to investigate a report of an explosion at a gas well. I arrived on scene and met Dustin Wyant, PA DEP Emergency Response and no one else on site. We found the well was leaking below the two inch valve on the tubing. I talked to David Rice (330-716-3967) and he reported the well-tender was on site earlier in the morning. After he left the well kicked on and the plunger traveled up hard and dislodged the lubricator from the two inch valve. Responders on site when the well-tender returned and shut the well in. The two inch valve threads were damaged and continues to leak a little. A crew is being scheduled to make repairs to the well. Mr. Rice of Bobcat Well & Pipeline will also secure the location to keep people away until repairs are. made. The Greenlee No.1 (085-23982) and Greenlee No.2 (085-23881) production units and brine tank share the same containment area (dike). Production was being recorded on the Greenlee No.1 While on site Anthony Sellards arrived on site and said he was the one

We found the well was leaking below the two inch valve on the tubing. Mr. Rice of Bobcat Well & Pipeline was scheduled to secure the location and make necessary repairs to the well. Todays inspection shows the backside tied in for production in attempt to feed the well down to replace the leaking valve. Needed parts are on site for repairs.
