Reviewed progress of drilling horizontal well. Running at 50 ft/hr. Patterson Rig 163. Current depth at 5,372 ft below grade at 11 % degree drilling vector. 11 3/4" pipe installed to 508 ft. with 150 sacks lead cement and followed by 150 sacks tail and 20 barrels circulated. 8 5/8" to 3490 ft with 710 sacks lead followed by 140 sacks tail with 90 barrels circulated. Inspected pit. No freeboard issues and pit is aligned with plastic. Small trench installed around drilling equipment to collect and releases. Slight droplets of oil located near storage bends. Collected in trench which was draining to pit.
1812917 Primary Facility 2009-07-15 Routine/Complete Inspection Violation(s) Noted A 20-30 gallon diesel spill was reported by Patterson UTI. A routine investigation was performed. The cleanup process was discussed with the safety officer, Sid Manning. The spill was contained and cleaned up using soap and pads. In addition a trench was utilized to contain the fluids within an area. There were no signs of residuals at the site. Well site has active drilling on location at the time of inspection. There were no signs of erosion and sedimentation on the well pad and access road. This is a Horizontal Marcellus well.
1821097 Primary Facility 2009-08-13 Compliance Evaluation No Violations Noted A CEI inspection was performed to evaluate the area in which a spill had occurred on 7/7/09. The incident was reported to the DEP. The spill was contained and cleaned up immediately using pads. There were no signs of residuals on the site at the time of the inspection. No signs of erosion and sedimentation at the well site. Pit maintaining freeboard. A large freshwater pit was being constructed at the time of the inspection.

Small trench installed around drilling equipment to collect and any releases. Samson in the process of completing frac pit located on the eastern portion of the site.
1837086 Primary Facility 2009-10-08 Routine/Complete Inspection No Violations Noted At the time of inspection, the well was drilled and prepared for hydro fracturing. The well was properly signed with E&S plan onsite. The access road was in good condition. No accelerated E&S issues were noted in relation to well site or access road. Pits sustained 2 ft of freeboard. It is recommended that pit freeboard continued to be monitored closely as areas of the pit were approaching the 2ft limitation.
1889551 Primary Facility 2010-06-02 Routine/Complete Inspection No Violations Noted At the time of inspection, well was drilled and in production. Site was properly signed with E&S plan onsite. Drilling was completed on August 20, 2009. Final restoration activities were underway during inspection. Pits were removed and backfilled, mulch and seed were being added at time of inspection. Well pad was rocked. I was informed by contractor, that farmer plans to utilize fields to perimeter around well pad, which will be seeded and mulched approximately 25-50ft from edge of rock pad. No accelerated E&S issues were noted in relation to well site or access road. Noted that two tanks were onsite and appeared sound. Dept requested that a container of methanol be removed from direct sunlight near pump station. A follow up inspection will be conducted to ensure final restoration activities have been completed and successful growth is achieved as well as removal of methanol barrel. Note: Operator noted that weather was a factor in timing of restoration activities.

Well owned by xtreme energy. Reason for inspection. Was a fire at heater located at cooling unit and compressor station. Heater malfunctioned. Causing heat to melt a gas line flange gasket. Fire was put out by Rj well services. Well was shut in. Damage to heater and plumbing line adjacent to heater. Remaining gas drained from line. Scott Sabocheck arrived at noon and joel keller at 12:15 pm. Review of the,site with supervisors. Departed at 13:15 hours.

The fire occurred at the production equipment and piping located approximately 135 feet from the Menhorn 2H well. At the time of the inspection on December 4, 2017, the well was temporary shut in. This Marcellus site has one active well (111-20272) and one plugged well (111-20270). Plugged well owned by another operator. Well appears equipped for production. Violations being issued for failure to have ERP on site, failure to have a site specific PPC plan, failure to conform with 78a55(5), failure to meet requirements of site ID sign, failure to install permit number on completed well, and primary containment used to store brine not structurally sound. Department requests operator immediately correct above issues and notify Department when corrected. A follow up inspection will be conducted at a later date.

Signs have been updated with new emergency numbers and well api numbers for the Menhorn 2 well. Well tag has been changed with proper api number. Dave Mathews present. Dave added new regulator and adding new heater and associated plumbing was replaced. The automatic shut off is ordered and will be installed on the well head.

2675999 Primary Facility 2017-12-28 Follow-up Inspection Outstanding Violations - Viols Req'd Follow up inspection to inspection record #2668350 where multiple violations were issued. At time of inspection site ID sign appears to have been updated. Well,tag present.

ERP, PPC plan, and well permit not on site. Department contacted operator rep Mike Hahn by phone from site. Mr. Hahn stated ERP and PPC plan have been updated and have been sent to Pennsylvania to be placed on site. Well tender Dave Walker on site during inspection. Mr. Walker stated he has not yet received updated ERP and PPC plan but will put them in site mailbox as soon as they arrive. As of this date, Department has not received operator response to NOV. Mr. Hahn stated operator response to NOV will be emailed to Water Quality Specialist. Violations issued on 12-1-17 for failure to meet requirements of site entrance sign and failure to install permit number on completed well have been corrected. Violations remain outstanding for failure to have ERP on site, failure to have a site specific PPC Plan, failure to conform with 78a55(5)

Operator rep Mike Hahn contacted Department by phone. Mr. Hahn requested clarification on ERP and PPS Plans and instruction on responding to NOV. Department responded to his requests by email.

At time of inspection, access road and site snow-covered. Well appears to be in production. Emergency Action Plan on site. Department requests operator ensure Oil and Gas Emergency Response Plan is on site and conforms with PA Chapter 78a regulations. Violations for operator failure to have ERP on site, ERP failure to include emergency contact numbers, and ERP failure to show location of fire suppression/spill control equipment have been corrected. In addition, violation issued for containment used to store brine not structurally sound has been corrected. Recommendations were suggested and outlined in an email sent by Department on 1-25-18 (photos 1-4). Department recommends operator notify Department when documents are finalized and on site.
