Running second geophysical logging event after hole was deepen from 8960 ft to 9123 ft. According to Chief, roughly 500 ft above Marcellus, Chief will fill hole up to that point with cement and set a kicker. Pit aligned with 20 mil plastic and no free board issues.

It is recommended that operator continue to maintain all bmps until final (uniform) restoration measures are achieved. Sub- contracting rep (Keystone) Josh was onsite during inspection. A follow up inspection will be conduction to ensure reseeded areas sustain 70% vegetative growth and site is permanently restored/stabilized.

Pipeline from Bowers West impoundment utilized for frac operator currently disconnected due to pipeline break that occurred on 2/20/11, (see incident follow up inspection # 1952923). No visible evidence that previous line break adversely impacted this site, at the present time. No violations noted onsite. Site had previously been reseeded and mulched, but operator should continue to monitor site/access to prevent the potential for accelerated erosion or sedimentation concerns due to recent thaws and increased traffic.

Two brine tanks located in secondary containment appear structurally sound. What appears to be two secondary containment liners remain on site. These liners are located on the east side of the pad and are draped over cement barriers. Department contacted Operator Rep Matt Gary by phone to inform Operator of residual waste left on site.

Violation is being issued for failure to dispose of residual waste.
