The operator should continue to monitor the access road and well site for erosion and sedimentation until final restoration activities are complete. This is a HORIZONTAL MARCELLUS CONSERVATION well.
1821130 Primary Facility 2009-08-13 Routine/Complete Inspection No Violations Noted The well is in the pre-drill phase. There is a pit present with a liner containing freshwater. The pit is maintaining freeboard requirements. There are no signs of erosion and sedimentation on the well site and access road. There is a well established vegetative filter strip surrounding the site. HORIZONTAL MARCELLUS.
1823464 Primary Facility 2009-08-25 Routine/Complete Inspection No Violations Noted The well is in the active-drill phase. There is a pit present maintaining freeboard requirement. There are no signs of erosion and sedimentation on the well site and access road. There is a well established vegetative filter strip surrounding the site. Investigated a possible spill at the site. The diesel spill that occurred was the result of a vehicle accident and did not occur on the well site. The spill occurred on the other side of route 31. The incident was contained and being cleaned up by Sugar Run environmental. Sugar run had a roll-off container on site that the contaminated soil was being loaded into to be hauled to an approved landfill. HORIZONTAL MARCELLUS.
1824675 Primary Facility 2009-08-26 Drilling/Alteration No Violations Noted At the time of the inspection, no E&S issues and no IW issues at the site. Well sign and E&S plan located at the entrance of the site. Paterson rig drilling well and was at 500 ft depth. Conductor pipe set at 81¿. 11 ¾¿ pipe set at 416¿ and used 175 lead sks and 150 tail sks. Pit aligned with 20 mil plastic and no free board issues. Spud date called on 7/17/09. DEP WQ inspectors (Chad Eisenman and Ed Ritzer) on-site to inspect second product release near the site along Rte 31. Sugar Run Spill Company excavating soil from vehicle accident. Sugar Run reported second release located in drainage pipe under Rte 31. After further investigation, black stain material in ditch coming from farm production and possibly manure runoff from farm spreading activity. During inspection, subcontractor transferring drilling pit fluids to Johnstown sewage plant for treatment.

Well sign and E&S plan located at the entrance of the site. Paterson rig drilling well and was at 8,770 ft depth. Conductor pipe set at 81¿. Surface pipe (11 ¾¿) pipe set at 416¿ and used 175 lead sks and 150 tail sks. First intermediate (8 5/8¿) set at 2152 ft with 390 sks lead and 145 sks tail. Pit aligned with 20 mil plastic and no free board issues. Spud date called on 7/17/09. During inspection, Samson had scheduled a subcontractor transferring drilling pit fluids to Johnstown sewage plant for treatment.
1845411 Primary Facility 2009-11-18 Routine/Complete Inspection No Violations Noted At the time of inspection, the well was properly signed with E & S plan. The well was drilled, not yet in production. No accelerated erosion or sedimentation issues were observed in relation to well site or access road. Compost socks were installed and functioning properly. One drill pit was onsite containing frac fluids and drill cuttings. Area surrounding the well site was reseeded with observed vegetative growth. Well pad was not yet restored.
1873442 Primary Facility 2010-03-24 Drilling/Alteration No Violations Noted At the time of the inspection, no E&S issues and no IW issues at the site. Well sign and E&S plan located at the entrance of the site. Samson contracted Well Superior to perform hydro frac on Marcellus Horizontal well. Pit aligned with 20 mil plastic and no free board issues. At the time of visit, frac was at the third stage. Representatives of MDOE were present to inspection hydro frac. Samson also showed the connection into transmission line and recording stations. At this location, no IW and E&S issues.
1884177 Primary Facility 2010-05-12 Routine/Complete Inspection No Violations Noted At the time of inspection, well was drilled and in production. Site was properly signed with E&S plan onsite. Pit maintained 2ft freeboard and appeared structurally sound, recommended operator continue to monitor pit freeboard levels. Filter sox¿s were properly installed around the site and functioning. Disturbed areas were temporarily reseeded. Access road was in good condition. No accelerated E&S OR IW issues were observed.
1888035 Primary Facility 2010-05-26 Routine/Complete Inspection No Violations Noted At the time of inspection, well was drilled and in production. Site was properly signed with E&S plan onsite. Pits maintained 2ft freeboard and appeared structurally sound. Samson Resource Rep, Charlie Howard was onsite and informed us that fluids were recently removed from frac pit, as pit levels looked significantly lower since last inspection. Filter sox¿s were properly installed around the site and functioning. Disturbed areas were temporarily reseeded. Access road was in good condition. No accelerated E&S issues were observed. Operator is awaiting department approval to solidify pit contents once all fluids are removed. Operator also filed for a 6month extension on site restoration. Accompanied by, Water Quality Specialist Supervisor, Ed Ritzer during inspection.
1897895 Primary Facility 2010-07-13 Routine/Complete Inspection No Violations Noted At the time of the inspection, no E&S issues and no IW issues at the site. Well sign and E&S plan located at the entrance of the site. Pictures of the solidification of pit at the Hillegass site. Based on my inspection, Samson has followed the proper procedure as outlined in 78.62 Disposal of residual waste and should continue with the site activity. Approximately 1800 sacks of cement has been used and mixed with the cuttings. Currently, Samson is going to place 3 ft of top soil on top of the pit upon completion of curing of the cement. Gas well was in production.
1927119 Primary Facility 2010-11-10 Site Restoration No Violations Noted At the time of this inspection, well site properly signed with ES plan. Well drilled and in production. According to WRACR well completed on 10/08/09. All pits and impoundments have been removed and area reclaimed. Site properly restored with >70% vegetative growth. No ES issues noted. Brine tank(s) sound. Access road stable. Recommend that operator place chemical barrels on/in a secure location.

Accompanied by Marc Maestra during inspection.

Well completed 10/08/09. Well site restored and maintained. No accelerated erosion or sedimenation issues with access road or well site. Temporary bmp's removed as well site has been permanetly restored with adequate vegetative growth and pad stabilized with rock. Prodcution equipment onsite, brine tanks located within metal containment dike. reccomend operator properly dipose of what appers to be "stormwater” that has collected in bottom of containment. During this inspection fluids present in containment dike were frozen. No industrial wastes issued noted. No violations noted during this inspection.

Department requests operator correct the above violations immediately. Please contact the Department when violations have been corrected. A follow up inspection will be conducted at a later date.

2676054 Primary Facility 2017-12-28 Follow-up Inspection Outstanding Violations - Viols Req'd Follow up inspection to inspection record #2668355 where multiple violations were issued.

ERP, PPC Plan and well permit not on site. Department contacted operator rep Mike Hahn by phone. Mr. Hahn stated ERP and PPC Plan have been updated and have been sent to Pennsylvania to be placed on site. As of this date, Department has not received operator response to NOV. Mr. Hahn stated operator response will be emailed to Water Quality Specialist. Violations issued on 12-1-17 for failure to meet requirements of site entrance sign and failure to install permit number on completed well have been corrected. Violations remain outstanding for failure to have ERP on site, failure to have a site specific PPC Plan on site, failure to conform with 78a55(5), and failure to display a sign on a container identifying the contents and showing an appropriate warning. A follow up inspection will be conducted at a later date.

Per operator VP of Operations Mike Hahn's request, contractor New Tech Global rep Walter Jenko contacted Department by phone. Mr. Jenko requested permission to send draft documents to Department for review. Department received DRAFT PPC plan and ERP via email from operator contractor New Tech Global. Deficiencies were found and outlined in an email sent to New Tech Global on 1-25-18. Department recommends operator notify Department when documents are finalized and on site. Previously issued violations still exist for this site.
