Temporary seeding done. Site Id sign was present with site name, but it contained an improper permit number. No E&S plan on site. BMP filter socks were incorrectly installed; stakes were not placed through the sock. Noted concerns about the construction of the pit (slope >2:1), and a corrugated pipe located on the site near the access road, which is absent from the plans submitted to the department. Access road drainage culvert pipe needs riprap outlets and ditches. Contacted operator representative ED Gray concerning theses issues awaiting a response when E&S plan will be on site and concerns addressed.

1836331 Primary Facility 2009-10-20 Drilling/Alteration No Violations Noted At the time of the inspection, no E&S issues and no IW issues at the site. Well sign and E&S plan was located at the entrance during was present. Noma Drilling Rig 8 was at 3308 ft depth. Conductor pipe (20¿) set at 35¿. 1548 ft of 10 ¾¿ pipe set with 1061 sks and 60 bbl return. Permit was on-site. Well log was present at the site. Stratigraphy consisted of sandstone in this section. No coal seams or methane detected nor any voids were detected during the inspection. Water present at 200 ft. Pit aligned with 20 mil plastic and no free board issues. Spud date called on 10/8/09. TD of well is 9031 ft depth and drilled to Marcellus and vertical well.
1840926 Primary Facility 2009-11-03 Drilling/Alteration No Violations Noted TD of well is 9031 ft depth and drilled to Marcellus and vertical well. During the inspection, pit showed signs of freeboard and industrial discharge outside of the pit area. The blow out area overtime has built a mound of drill cuttings outside of the pit area. I informed the site supervisor to clean up the problem within 24 hrs of the next DEP inspection. On 11/4/09, DEP called Trent form Chesapeake to inform of the freeboard and industrial discharge problem.
1845360 Primary Facility 2009-11-05 Routine/Complete Inspection No Violations Noted This inspection was conducted as a follow up from an inspection (Id # 1840926) done by Inspector Paul Wyrwas on 11/03/2009, concerns with pit freeboard and industrial discharge were noted during his inspection and he issued a 24hr notice to address the concerns. At the time of this inspection, active drilling of the well was occurring. Well site was properly signed and E&S plan onsite for review. Access road was in stable condition and no accelerated erosion or sedimentation issues were observed in relation to well site or access road. Pit freeboard was restored and drill cuttings were placed back in to the pit. It is highly recommended that operator continue to monitor 2ft of freeboard closely on drill pits at all times and install a baffle on drill pipe in an effort to eliminate possible pollution events in the future.
1845412 Primary Facility 2009-11-18 Routine/Complete Inspection Violation(s) Noted At the time of inspection, the well was drilled and properly signed with E&S plan onsite. No accelerated erosion or sedimentation issues were observed in relation to well site or access road. Access road was in stable condition with 50ft rock entrance, and well pad was nicely rocked. Two ft pit freeboard was exceeded on drill pit and drill cuttings had been discharged outside of the pit (industrial discharge) on to well pad. Mulching the discharge (as noted in pictures) is not an approved method of disposal/cleanup. Again, it is highly recommended that the operator carefully monitor the pit freeboard closely and install a baffle on drill discharge pipe in an effort to eliminate possible pollution events as described above.
1850827 Primary Facility 2009-12-10 Compliance Evaluation Outstanding Violations - No Viols Req'd At the time of inspection, well was drilled not yet in production. Rig was onsite preparing for fracing. It appeared pit freeboard had been restored, but industrial discharge of waste onto the ground had not been addressed, as it was still covered with mulch. No new violations were observed, one of the two violations were corrected (pit freeboard). To date, I am awaiting operator response on corrective action plans.
1911772 Primary Facility 2010-09-08 Plugging(Includes Plugged/Mined Through) No Violations Noted At the time of the inspection, no E&S issues and no IW issues at the site. Well sign and E&S plan was located at the entrance during was present. Chesapeake contracted drilling company to plug well. The company was setting up the guide wires at the time of inspection. Pit was aligned with plastic. Reviewed plugging plan with driller. Bridge plug was going to be set on Friday. Well had 2200 psi head pressure
1912795 Primary Facility 2010-09-13 Drilling/Alteration No Violations Noted At the time of the inspection, no E&S issues and no IW issues at the site. Well sign and E&S plan was located at the entrance during was present. Chesapeake contracted Hydrocarbon Well Services drilling company to plug well. The company was set bridge plug at 8650 ft. and cement column on top of plug. Pit was aligned with plastic. Frac sand was pushed to the edge of pit and has fallen into pit and along the side of pit wall. DEP requested that sand be removed from the pit side wall and placed into pit to prevent a freeboard issue. Reviewed remaining plugging plan with driller and has scheduled completion of plugging project by 9/16/10.
1919991 Primary Facility 2010-10-12 Routine/Complete Inspection No Violations Noted Met onsite with Jim Johnson and Kevin Swiger of Chesapeake Energy to review final site restoration plans. Operator intends to remove all pit contents (liquid, cuttings, and liner) and transport to an approved land fill. Dept also recommended that areas surrounding the pit that may have been or appear to be impacted by drill cuttings should also be transported to an approved facility rather than incorporating into site. Site is to be restored as close to pre existing contours as possible. Additional bmp¿s (filter fence/compost sox) will be installed prior to restoration activities commencing and remain until >70% uniform vegetative growth is observed across the site. No accelerated ES issues observed. Operator plans to have pits removed, and site regraded and seeded/mulched within next several weeks. Operator was also notified that if optimal growth (70% uniform veg. cover) is not achieved prior to first frost/snow, reseeding and mulching may be required in the spring in order to achieve permanent final restoration.
1941147 Primary Facility 2011-01-05 Bond Release No Violations Noted At the time of inspection, well plugged and marked with vented erected monument with permit number affixed to plate. All pits have been removed and areas regarded. According to operator, (Jim Johnson) stated that all pit contents were hauled off to landfill. Site been seeded and mulch but <70% vegetative growth observed. Follow up inspection will be conducted to ensure that adequate uniform vegetative growth is sustained/maintained, as required by regulation. Temporary bmp¿s are still in place until final permanent restoration (>70% permanent vegetative cover) is achieved. No accelerated erosion or sedimentation issues observed. Rock access road and pad have been successful removed regarded and seeded/mulched as well. Accompanied by WQS Marc Maestra during inspection.

Site been seeded and mulch >70% vegetative growth observed. No accelerated erosion or sedimentation issues observed. Rock access road and pad have been successful removed regarded and seeded/mulched as well. Site adequately restored. Reminder, hence final permanent stabilization/restoration is achieved operator may remove temporary bmp's in place (ie. compost socks, brush grub). No violations noted during this inspection.
