1889521 Primary Facility 2010-06-02 Routine/Complete Inspection No Violations Noted At the time of inspection, well was not yet drilled. Patterson UTI was arriving onsite and setting up for drilling. Site was properly signed with E&S plan available for review. Access road appeared stable with sufficient gravel. All disturbed areas were temporarily seeded and mulched. Spoke with Lou of Chief about reinforcing filter sox with stakes, it was noted farmer had previously moved the filter sox¿s, and Chief was in the process of replacing in original position. No accelerated E&S issues were noted in relation to well site or access road. Also noted that area beneath rig was being plastic lined. Also Lou noted they are utilizing frack tanks at this site, first site in area with which they are doing so (NO FRAC PIT). Freshwater impoundment was properly lined and was being mulched/seed during inspection, pit was not full yet. Anticipates that drilling would commence in 2-3 days. (NOTE: ESCGP-1 SITE)
1889979 Primary Facility 2010-06-08 Drilling/Alteration No Violations Noted Spud called on 5/27/10. Drilling started on 6/4/10. Patterson #255 contracted to drill horizontal Marcellus well. Site ID sign located along Rte 281 and entrance to access road. E&S plan present. According to WQ personnel, April Wieland, filter socks were moved by landowner to mow the field near the site pad. However, the socks were not properly re-installed and staked into position as indicated by the E&S plan. DEP called Lou Shaw from Chief about the issue. Current depth was 1746 ft. Conductor pipe was installed at depth of 72 ft. and surface pipe was set at 400 ft. with 309 sacks of cement and 15 bbl return. Permit and log present at the site. Closed loop system with the cuttings going to Apex Landfill in Ohio. 30 mil plastic located underneath rig and closed loop system. At the time of inspection, no E&S or IW issues.
1893491 Primary Facility 2010-06-18 Drilling/Alteration No Violations Noted Spud called on 5/27/10. Drilling started on 6/4/10. Patterson #255 contracted to drill horizontal Marcellus well. Permit and log present at the site. Closed loop system with the cuttings going to Apex Landfill in Ohio. 30 mil plastic located underneath rig and closed loop system. At the time of inspection, no E&S or IW issues.
1899068 Primary Facility 2010-07-09 Routine/Complete Inspection Violation(s) Noted During inspection well site was properly signed with E&S plan and ESCGP-1 onsite. Operator reported a spill (on 7/7/10 at 1600hrs) of approx 30 gallons of oil based mud. Violations will be issued for the release of pollutant onto the grounds of the commonwealth. According to operator, pollutant material was immediately excavated off ground, placed on felt onto a plastic liner which was enclosed with a liner on top of well pad, as seen today. (See Pictures). At time of inspection, material had not yet been transported to a facility nor was it secured from weather (ie runoff). While onsite spoke with contractor rep Bob Kennell, who was awaiting a dumpster to transport materials to an approved facility. Otherwise dept recommended that a pit be dug and lined accordingly, then place materials (as is in liner) in until proper disposal can be achieved in order to prevent runoff concerns. A follow up inspection is planned to ensure pollutions were disposed of properly and disturbed areas have been properly restored.
1927860 Primary Facility 2010-11-10 Routine/Complete Inspection No Violations Noted At the time of this inspection, site signed with ES plan and ESCGP1. Chief completion foreman, indicated well was recently flared. Frac tanks present onsite. Observed that pad is covered with a plastic and felt liner. No IW issues noted. Site has been seeded/mulched with slightly less than 70% vegetative growth observed. No accelerated erosion or sedimentation issues observed. Observed that sediment trap contains water. Recommend that operator continue to routinely monitor all bmps. Access road stable and adequately rocked. Site is monitored by security 24hrs a days.
1953260 Primary Facility 2011-02-23 Routine/Complete Inspection No Violations Noted At the time of inspection, site ID sign, ESCGP-1, and Es plan present. Site secured with fence and gate to prevent unauthorized acts of third parties. Well drilled not yet in production. Containment felt located on pad surrounding well head. One tank located on site. No IW discharges observed. No accelerated erosion or sedimentation issues observed in relation to site or access road. Appears site has been seeded and mulched, some growth observed. ES controls in place and functioning. Recommend that operator continue to routinely monitor all bmps (ie sediment traps) until final permanent stabilization/restoration achieved. Access road stable and adequately rocked. Site and pad partially snow covered.
1965778 Primary Facility 2011-04-14 Routine/Complete Inspection No Violations Noted At the time of inspection, site Id sign, ES plan, and ESCGP-1 onsite. Access road stable. Site is protected from third party access with fencing and locked gate. Well has been drilled not yet in production. Observed large sand pile on pad, recommend operator remove in order to prevent a possible pollution event. Also observed large quantity of PVC pipe located on site possibly for later use during fracing operations. Bmp¿s installed and functioning according to plan, operator should continue to monitor/maintain all temporary bmp¿s until final permanent site restoration is achieved¿including sediment traps (basins). No accelerated ES or IW issues observed. Previously disturbed areas surrounding well site have been seeded/mulched with observed growth.

Well drilled not yet in production¿pipeline project underway adjacent to well site, well recently granted inactive status. Previously disturbed areas seeded/mulched and >70% vegetative growth observed. No accelerated erosion or sedimentation issues observed. Although noted a large soil and sand pile located on the upper left hand side of well pad (when facing well from access road entrance) that department recommends operator remove in order to prevent a possible pollution event. Two brine tanks located onsite¿located in a lined containment unit. Appears rainwater has begun to accumulate in containment unit due recent heavy rains; department is recommending operator properly dispose of water from unit. No violations noted during this inspection.

Operator shall continue to closely monitor and properly dispose of collected water located within dike. Pad adequately rocked. Previously disturbed areas surrounding the site have been seeded/mulched with adequate growth. Temporary bmp's removed. No accelerated erosion or sedimentation issues noted. Shared access road stable, again no accelerated erosion or sedimentation issues noted. As noted to Chevron reps in an early inspection, large soil and sand pile located at north eastern corner still exists has not yet been removed. In order to prevent a possible pollution event again department recommends operator properly dispose of piles. No violations noted during this inspection

Three unmarked barrels (approx. 42 gal each) are located on well pad near brine tanks. These barrels are not in secondary containment. Department contacted Operator Rep Matt Gary by phone from site. Department requested follow up call from Operator as to the contents of the 3 unmarked barrels.

Violations may be issued pending Operator response.

UPDATE 2-13-17: Department contacted Operator Rep Clayton Hill by phone to inquire about contents of unmarked barrels. Mr. Hill stated 2 of the barrels contained spill kits and one was empty. Inspection Result being updated to ICP; Operator appears to be In Complia
