Second stringer was 13 3/8﾿ installed to 462 ft with 330 sks of gas blocks with 11 bbl return. Current depth was 1781. At the time of inspection, no E&S issues or IW issues at the site. Chief proposing to install 9 5/8﾿ pipe to 1811 ft.

Well spudded 6/2/11. Well drilled. Access road/entrance and well pad adequately rocked. BMPs installed and functioning according to ES Plan and appear properly maintained. No accelerated erosion or sedimentation issues noted during inspection. Previously disturbed areas have been seeded/mulched with adequate growth observed. R Miller 1H and 6H spudded and drilled, although yet in production. No pits or tanks present on site. Miller pipeline propject actively being installed. No violations noted during this inspection.

2069379 2012-05-10 No Violations Noted At the time of this inspection, site Id sign, Es plan, ESCGP-1, and PPC plan onsite. Site gated and monitored by 24hr security. Multi well pad. Said well previously drilled. Crown Rig # 8 actively drilling 6H. Charles, DSM onsite during this inspection. Secondary containment in place beneath rig and onsite tanks. No pits installed by Chevron for drilling operations. Tanks and secondary containment look structurally sound. Appears bmp's are installed and functioning according to plan. No accelerated erosion or sedimentation issues noted with site or access road. Access road adequately rocked.Noted a "small" leak coming from Residual waste box on site, notified DSM whom stated the problem would be addressed immediately. Absorbent pads were placed in location of leak. Department also noted to DSM of a possible safety concern near back side of rig, where it appears shale was placed over top electric lines and drilling equipment (see picture). DSM stated that matter would be addressed immediately no violations noted.

(20ï¾’ï¾’pipe) and was grouted in place. 13 3/8ï¾” pipe was set at 457ï¾’ (surface pipe) and the 9 5/8ï¾” pipe was set at 1807 feet. Chevron current depth was 5,663 feet. Chevron was planning to TD at 7500 feet and set the kick off point. DEP personnel performed a site walk and did not find any issues.

(20ï¾’ï¾’pipe) and was grouted in place. 13 3/8ï¾” pipe was set at 457ï¾’ (surface pipe) and the 9 5/8ï¾” pipe was set at 1807 feet. Chevron current depth was 14,255 feet. Chevron set the kick off point at 7829 ft. DEP personnel performed a site walk and did not find any issues.

2141760 2013-02-19 In Compliance with Policy Admin Inspt. Spoke to Chevron representative B.A. Lambie via phone on 2/19/13. According to Mr. Lambie, on 2/18/13 as truck was loading production brine, truck tank began leaking due to failed tank seam. Resulted in approximately 10 gallons of brine released to ground, impacting an approximate area of 15 square feet. Impacted materials immediately excavated into drum. Samples taken, will dispose of in landfill. It appears Operator handled situation according to policy and a follow-up inspection will be conducted in future to ensure full compliance.
2145150 2013-02-25 No Violations Noted Regulatory inactive status. Multi-well pad.

Site ID sign, permit, E&S plan, ESCGPI, PPC plan all on-site. At time of inspection, 2H and 1H, 6H drilled and equipped for production. Brine tanks on steel containment, both appear structually stable. Site temporarily stabilized. Access road stable, adequately rocked. No accelerated E&S issues observed in relation to well pad and access road. According to records, as of 8/21/2012, 3H, 4H, and 5H were all proposed, but never materialized. Department recommends that Operator update site ID sign to ensure that only wells with active permits are listed.

2196932 2013-08-20 No Violations Noted Inspected the site on 8/20/13. There was not any activity on the restored well pad. The road going into the site was well graveled. The welll pad was well graveled. The Well pad signs were properly marked. This pad has three producing Marcelus gas wells. The three wells were properly marked.The drums observed in the past inspection were removed from the pad.There were two dumpsters on the well pad that contained Demo waste. No staining on the well pad was observed. The fresh water impoundment is now fenced in. No E&S issues were observed. During the inspection no issues were observed that need to be addresed
