Present at time of inspection was Bryan Stevens (Conserve) and Warren Schoenfelt (Shell Consultant). The site is located in Blaine Twp., Washington Co. The inspection was related to a reported release, by the operator, of 3-5 barrels of crude/water mixture to the ground on 8/16/17.

The release was reported to the Harrisburg office of the Department within 2 hours of the release.

While pumping a cement plug into the wellbore, the displacement water from the annulus was flowing into a wellhead cellar (sump) for capture and pumped out to a vacuum truck. During the operation, the vacuum pump temporarily failed and approximately four to five barrels of a crude oil/water mixture was displaced from the cellar and released onto the well pad (estimated 0.5 barrels of oil released with brine). The cause of the release is attributed to the failure of a vacuum truck pump due to debris blocking the suction.
