There is a vent pipe approximately 1 ½” in diameter attached to the well head. Well is connected to an upright, green tank about. Observed several gallons of light brown fluid (appeared to be brine/crude oil mix) in two areas at base of tank which appears to be leaking from the tank. There is no dike around tank. Also observed dead vegetation around tank. There is a hose, approximately 1” in diameter, connected to the base of the tank going to a small pond behind and downhill from the tank which then flows through a small trench into the brush.

2425461 Primary Facility 2015-08-27 Follow-up Inspection Pending Follow up inspection after receiving no response from operator on four (4) violations sent 4/8/2015. Oil-saturated soil remains around well head. Well is connected to an upright, green tank about 100 yards across Langleloth Road. There is no dike around tank. Also observed dead vegetation around tank. There is a hose, approximately 1” in diameter, connected to the base of the tank going to a small pond behind and downhill from the tank which then flows through a small trench into the brush. There is also a second green tank beside the upright tank, on its side, which is now connected to the upright tank. That appears to be the only change since last inspection. There has been no production reported on this well and it appears to be orphaned.

Met with environmental company representative coordinating cleanup and Department’s Environmental Protection Specialist and Water Quality Specialist. All soil testing is complete and site of excavation can now be back filled.

Operator has yet to back fill site. Excavated area around well head remains open creating a dangerous situation. Only caution tape around excavated area approximately 5 ft. deep. Concrete pier and wooden beam have not been removed.

The operator shall remediate all impacted soil. Please retain all waste manifests and receipts for work done at this well.

Area around wellhead has been backfilled. Backfilling is rough with large clumps of clay and rocks and open areas creating a hazard. Concrete pier with large wooden beam was to be removed but remains. Operator has yet to backfill site of collection tanks across road from Scott 10 well. Only caution tape around excavated area creating a hazard. No production has been reported for this well.

Please retain all waste manifests and receipts for work done at this well.
