The inspection was conducted to evaluate work completed to date which has been conducted in response to a September 20, 2016 inspection of Hixenbaugh 1 (125-22448), Hixenbaugh 2 (125-22788), Hixenbaugh 3 (125-22847), Hixenbaugh 4 (125-22848), Hixenbaugh 5 (125-22849) and Hixenbaugh 6 (125-22850). The inspection resulted in violations of 25 Pa. Code Section 78.57(a) (78.57(a)), 35 P.S. Section 6015.301 (SWMA 301), 25 Pa. Code Section 91.33 (91.33), 25 Pa. Code Section 91.34(a) (91.34(a)) and 25 Pa. Code Section 102.22(a) (102.22(a)) being sent to K Petroleum, Inc. (KPI) for deficiencies identified during the inspection (Insp. No. 2532594, Viol.
