2582953 Primary Facility 2017-02-22 Complaint Inspection No Violations Noted On 2/22/17, a complaint inspection was performed related to the Latkanich site in response to complaint 325171 and made the following observations:

•The complainant requested testing for radiological compounds as reportedly recommended to them by the Isaac Walton League. •The complainant reported suspected past improper disposal of fluids in former ponds on site. Previous inspections of site found no surface indications of spills or contamination. •Samples for 946 and bacteriological analysis were obtained from the tap before the complainants holding tank before any filtration after a 10 minute flush of the system. The sample appeared clear and no smell was noted (possibly due to strong odors present in basement). •The well bore is located roughly 20 feet from the house and has a secure cap elevated 6” off of the ground at the time of inspection. The complainant reported that the well was drilled to 275 feet deep in 1999 and has a poly case to 70 feet.

2585316 Primary Facility 2017-04-20 Administrative/File Review No Violations Noted On 4/20/17, an ADMIN inspection was conducted assisted by WQS Supervisor John Carson regarding the Latkanich site in response to complaint 325171 and made the following observations:

•Results from operator predrill samples taken 8/2/11 and post drill samples from 3/26/13 and 4/8/13 were analyzed in comparison to DEP samples obtained during inspection 2582952 on 2/22/17. Increases in levels of multiple parameters were noted but no conclusive indicators of oil and gas impact were observed.

2660489 Primary Facility 2017-11-08 Follow-up Inspection No Violations Noted On 11/8/17 an FUI inspection was conducted of the Latkanich well site in response to a complaint (CTS ID: 325171) accompanied by WQSS John Carson and DEP Dave McDermott. The following was observed:

>>Labeling on the production tanks was noted to be weathered and difficult to read.

>>Containment around a container labeled residual waste was found to be partially compromised and unlikely to hold sufficient liquids in the case of a spill or leak. Further information concerning the contents and providence of the tank have been requested from the operator.

>>Production equipment on site was noted to have developed substantial rust. Protection from further weathering and potential failure is encouraged.

>>The complainant reported ongoing concerns of impact to well water quality and damage to the home. Sample 3401-068 was collected for 946 and bacteriological analysis.

2774930 Primary Facility 2018-09-05 Administrative/File Review No Violations Noted On 9/5/18 an administrative inspection of the Latkanich Unit Well site conducted with Group Manager Dave McDermott and WQSS John Carson in response to ongoing investigations into site conditions. The following was found:

>>Multiple areas of the site including sections of the entrance, access road and pad were found to have been constructed contrary to permitted plans. This constitutes a violation of 25 Pa. Code § 78.53, 25 Pa. Code § 102.5(c), and 25 Pa. Code § 102.5(m)(4) in that the operator failed to construct to comply with permit conditions in constructing the site and failed to acquire required permits or permit modifications to alter the site from permitted plans.

>>Multiple areas of the site including sections of the entrance, access road and pad were found to have been constructed contrary to permitted plans. This constitutes a violation of 25 Pa. Code § 78.53, 25 Pa. Code § 102.5(c), and 25 Pa. Code § 102.5(m)(4) in that the operator failed to construct to comply with permit conditions in constructing the site and failed to acquire required permits or permit modifications to alter the site from permitted plans.

2807705 Primary Facility 2018-10-30 Complaint Inspection No Violations Noted On 10/30/18 a complaint inspection related to the Latkanich Unit was conducted in response to a water supply complaint (CTS ID: 332897). Water samples were collected for 946 and radiological compound testing (the latter were sent to a third party lab for analysis).
