2815074 Primary Facility 2018-11-27 Routine/Complete Inspection No Violations Noted Today I conducted a routine inspection at the NV41HS well pad and the following was observed.

Proper signs were displayed identifying the site. The access road was maintained and in good condition. The outer slopes were stabilized with thick vegetation no erosion observed outside of the LOD. The well heads were maintained no leaks or defects observed. The production tanks were in good condition no defects or leaks.The production tanks secondary containment was intact.


GPU did not have secondary containment. The well was drilled and developed in 2011-2012.
