2164915 2013-05-09 In Compliance with Policy An Administrative Review of this incident revealed the following:

On May 9, 2013, the department received an email report form Nick Staffieri about a small sewage spill at Chevronï¾’s Bercosky 6 site in West Bethlehem Township, Washington County. The email was a follow-up of a report to Vince Yantko on May 7th.

According to the report, a vent overflowed, resulting in a small (6 gallon) sewage spill. The restroom trailer was taken out of service and thesite was cleaned appropriately.

Upon review of this spill incident, Chevron met the reporting requirements and the reported remediation is in compliance with the Departmentï¾’s current Spill Policy.

2172607 2013-06-06 Violation(s) Noted On May 15, 2013, Chevron had a brine spill at its Bercosky site located in West Bethlehem Township, Washington County. According to Chevronメs incident report, a driver spilled about 50 gallons of reuse water during unloading. Although the incident happened at approximately 10:00, Chevron did not notify the Department until 15:30; this is a violation of ᄃ 78.66(b) of the Rules and Regulations.

Chevronï¾’s report indicates that they handled the rest of the incident correctly.
