2288018 Primary Facility 2014-06-11 Routine/Complete Inspection No Violations Noted Tank overflowed into secondary containment (<1 bbl) - reported as a spill but all material was captured in containment and no flowback contacted the ground. Current operations - drilling out the plugs in each well (AHS on this day). No violations noted during inspection of the site.

2455252 Primary Facility 2016-02-25 Routine/Complete Inspection No Violations Noted A routine complete inspection was conducted on Thursday, February 25, 2016. Filter sock was observed around the southern perimeter of the well pad and also mid-slope in the northern area of the site which should be removed given that the ESCGP-1 was terminated July 8, 2015. An infiltration berm constructed according to the site’s PCSM Plan does not appear to be functioning properly. A pond of water has formed behind the berm. At the time of inspection, the water level within the berm was at a maximum and was observed to be slightly overflowing the top of the berm. Signs of more significant past overflow events were observed from Harvey Road which suggests that subsurface materials may not be permeable enough to allow for sufficient infiltration. An inquiry will be made to the SWRO Permitting Section to evaluate design adequacy. No violations are being cited at this time.
2692201 Primary Facility 2018-02-08 Routine/Complete Inspection No Violations Noted On Thursday, February 8, 2018, I conducted a routine-complete inspection at the NV56 Well Site located in Morris Township, Washington County.

The well, NV56CHS (125-24835), and the associated infrastructure appeared to be in good working order and appeared to be functioning properly. The well was labelled.

The operator should continue their periodic monitoring program and address deficiencies upon identification.

No violations were identified during the inspection.
