2476326 Primary Facility 2016-04-20 Administrative/File Review No Violations Noted On 4/20/16, I performed an administrative review of sample results and site observation for a complaint related to the Sargent site and made the following observations:

•Water samples were found to have elevated hardness and iron levels; coliform and e. coli bacteria presence was also found indicating surface water influence. •Based on distance to nearest wells (~3000 and ~5000 feet away) and dates of most recent activity (4/19/15 and 9/4/10) as well as the above test results no oil & gas activity impact is indicated.

2593528 Primary Facility 2017-05-15 Routine/Complete Inspection No Violations Noted On May 15, 2017, I conducted a routine-complete inspection of the Sargent Well Site located in West Bethlehem Township, Washington County.

The Act 9 sign was in place and clearly legible at the site entrance which is secured with a locked gate. The well pad is also secured with a separate chain-linked fence and lock. The access road and the well pad are in good condition and the site is stabilized with vegetative growth.

The secondary containment structure around the production tanks appears to be in good condition and functioning as intended. The gas wells appear to be in good working order and are labelled accordingly.

No violations were identified during the inspection.
