2343098 Primary Facility 2015-02-08 Drilling/Alteration No Violations Noted On site to witness planned remedial 5 1/2" casing cementing job. Job delayed until Monday AM, waiting on additional cement as part of contigency planning. NOVIO
2343104 Primary Facility 2015-02-09 Drilling/Alteration No Violations Noted The remedial squeeze was performed on the Sargent 15H today. There were delays to ensure everything was in place for contingency plans in case of a failed cement job. Chevron did a very good job planning for the squeeze.
2344395 Primary Facility 2015-02-01 Drilling/Alteration No Violations Noted Check remdial work on 15H. Retrieved fish last night, ROH on coiled tbg, today run and retrieve swadge. Tonight run sleeve and then mill top of sleeve. Tomorrow AM anticipated PSI test. Tomorrow PM anticipated start of stimulation on three adjacent wells. Told WSM to give me 4 hours notice for frac job. NOVIO
2344958 Primary Facility 2015-02-18 Drilling/Alteration No Violations Noted Setting casing patch. Problems setting top rubbers. Will set patch, mill top of patch, PSI test and notify OGI 4 hrs in advance of anticipated stimulation. NOVIO
2344964 Primary Facility 2015-02-19 Drilling/Alteration No Violations Noted Casing patch set. Milled off top of liner. PSI test to 9900 PSI succesful and documented. Rigging down coil tbg unit and moving off location. Anticipated stimulation this PM or early tomorrow AM. WSM notified to give DEP 4 hrs notice by phone before frac job. WSM notified that requirement for continuing with stimulation requires separate monitoring of the 15H annulus during stimulation and recording with all stimulations for permanent record and real-time data evaluation. NOVIO
2345604 Primary Facility 2015-02-20 Drilling/Alteration No Violations Noted Chevron was able to stimulate the first zone in an adjacent well to the Sargent 15H this evening. There were no anomalies. There was no communication with the annulus on the 15H. The WSM's have made it clear to Schlumberger that if communication with the 15H is observed they should shut down immediately, in emergency mode, and await further instructions. They have also made it clear that all fracs performed on the Sargent Pad will record real-time data of the 15H annulus. NOVIO
2345620 Primary Facility 2015-02-21 Drilling/Alteration No Violations Noted During Frac operations of Sargent 9 -H ( 3 rd stage) there was failure of transducer on Sargent 15 - H . Pressure were from 0 -150 psi. Continue Frac operation with no issues.
2346242 Primary Facility 2015-02-23 Drilling/Alteration No Violations Noted Inspected manifolding for monitoring annulus of 15H. Having freezing issues may be affecting transducer readings. Updated monitoring to Schlumberger and Chevron on data display and data reporting. Ready to frac. NOVIO
2346243 Primary Facility 2015-02-24 Drilling/Alteration No Violations Noted Still having difficulty with freezing in manifolding. Ready to frac. Asked for ASCII data on all stimulation jobs to date and anticipated frac schedule. NOVIO
2347726 Primary Facility 2015-02-26 Drilling/Alteration No Violations Noted Currently stimulating 7H, 8H and 9H while monitoring annulus on 15H. NOVIO
2352595 Primary Facility 2015-03-18 Routine/Partial Inspection Violation(s) Noted § 78.86. Defective casing or cementing.
2355737 Primary Facility 2015-03-27 Routine/Complete Inspection No Violations Noted On Friday March 27th 2015, I inspected the Sargent site in response to an extension request and made the following observations:
2356583 Primary Facility 2015-03-31 Drilling/Alteration No Violations Noted All frac jobs completed on Sargent well site. WSM indicated no communication with annulus of the 15H noted. Special care taken while monitoring annulus on 15H on the adjacent lateral sections. Moving frac crew off location by end of week. NOVIO
2441444 Primary Facility 2016-01-19 Compliance Evaluation No Violations Noted This is an administrative CEI.

As per Chevrons response and numerous on site inspections by the department, operator has provided the required plan for remediation, performed the remedial cement job and subsequent monitoring of the well during adjacent frac jobs show the well appears to be properly isolated with cement. Further review after writing the initial violation shows that the operator did indeed notify the department and the violation for failure to notify of adefective cement job is incorrect and the violation for failure to notify is withdrawn. NOVIO

2476243 Primary Facility 2016-03-24 Complaint Inspection No Violations Noted On 3/24/16 water samples were collected for SAC 946 and bacterial count analysis in response to a complaint regarding the Sargent site.
2476333 Primary Facility 2016-04-20 Administrative/File Review No Violations Noted On 4/20/16, I performed an administrative review of sample results and site observation for a complaint related to the Sargent site and made the following observations:

•Water samples were found to have elevated hardness and iron levels; coliform and e. coli bacteria presence was also found indicating surface water influence. •Based on distance to nearest wells (~3000 and ~5000 feet away) and dates of most recent activity (4/19/15 and 9/4/10) as well as the above test results no oil & gas activity impact is indicated.

2593537 Primary Facility 2017-05-15 Routine/Complete Inspection No Violations Noted On May 15, 2017, I conducted a routine-complete inspection of the Sargent Well Site located in West Bethlehem Township, Washington County.

The Act 9 sign was in place and clearly legible at the site entrance which is secured with a locked gate. The well pad is also secured with a separate chain-linked fence and lock. The access road and the well pad are in good condition and the site is stabilized with vegetative growth.

The secondary containment structure around the production tanks appears to be in good condition and functioning as intended. The gas wells appear to be in good working order and are labelled accordingly.

No violations were identified during the inspection.
