2580152 Primary Facility 2017-04-07 Routine/Complete Inspection No Violations Noted On 4/07/2017, WQS Brian MacQuarrie onsite at unconventional well site. Seven wells in production. Six unconventional Marcellus wells, and one well with pump jack setup. Well Permit No. 37-125-27681 (14 HS) was issued on 7/06/2016. Two tank batteries onsite. Tank battery 1 consists of two 400 bbl tanks installed within stainless steel containment structure. Tank battery #2 consists of six 500 bbl tanks installed within stainless steel containment structure. Act 9 sign installed and maintained. Erosion and Sedimentation Best Management Practices installed and maintained. Vegetative growth established in previously disturbed areas. No apparent E/S issues.
2769732 Primary Facility 2018-08-30 Routine/Partial Inspection No Violations Noted On 8/30/2018 at approximately 0915, I conducted a routine partial inspection of the George Godwin Well Site located in Cross Creek Township, Washington County. I met with Hannah McAvoy at the site to discuss a proposed addition of an access road off the pad at the request of the landowner.

The plan was to install a culvert in the drainage channel and cover it with stone to allow the landowner to access areas of land to the east of the pad. The proposed plan would allow unimpeded flow of the drainage channel while allowing the landowner to access the land without disturbing the existing channel. The plan was determined to be acceptable and a minor modification to the permit was redlined in the field.

6 unconventional marcellus wells and one well with a pump jack setup were on site and appeared to be in good condition. 2 sets of production fluid/condensate tanks were on site and appeared to be in good condition. The Act 9 sign located at the site entrance was in good condition. All PCSM BMPs on site appeare

2779754 Primary Facility 2018-09-12 Routine/Complete Inspection No Violations Noted On 9/12/2018 at approximately 1200, I along with John Carson (DEP) conducted a routine site inspection of the George Godwin Well Site located in Cross Creek Township, Washington County in relation to a complaint investigation that was being conducted that day. The following was observed:
  • The Act 9 sign located at the site entrance appeared to be in good condition.
  • The GPUs on site were not in secondary containment but appeared to be in good condition. GPUs were installed prior to October 2016.
  • Production fluid/condensate tanks on site were within secondary containment and appeared to be in good condition.
  • All wells on site were clearly marked and appeared to be in good condition.
  • A gas lift compressor was in operation at the time of inspection.
  • The site appeared to be stable with no signs of erosion.

No violations were found during this inspection.

2801255 Primary Facility 2018-11-07 Routine/Complete Inspection No Violations Noted On 11/7/2018 at approximately 1100, I conducted a routine inspection of the George Godwin well site located in Cross Creek Township, Washington County. I met with Hannah McAvoy (Range Resources) to assess site conditions relative to permit termination requirements. The following was observed:
  • Act 9 sign located at the site entrance was in good condition.
  • Production fluid/condensate tanks were within secondary containment and appeared to be in good condition.
  • GPUs on site were not within secondary containment but appeared to be in good condition. Wells on site were put into production prior to October 2016.
  • All wells on site were marked and appeared to be in good condition.
  • A gas lift compressor was being utilized on site and appeared to be in good condition.
  • The site overall appeared to be stable with no signs of erosion. One slope was stabilized with R3 rather than vegetation and appeared to be stable.

All requirements relating to permanent stabilization and permit termination appear to

2811730 Primary Facility 2018-11-30 Routine/Complete Inspection No Violations Noted On 11/30/2018 at approximately 0900, I conducted a site restoration inspection of the George Godwin Well site located in Cross Creek Township, Washington County to assess site conditions relative to permit termination. The following was observed:
  • The Act 9 sign located at the site entrance was in good condition.
  • 7 wells equipped for production were on site. Preparations were being made by the operator to drill additional wells on the pad including the construction of a sound barrier during the inspection.
  • Production fluid/condensate tanks were within secondary containment and appeared to be in good condition.
  • All wells on site were clearly marked and appeared to be in good condition.
  • GPUs on site appeared to be in good condition.
  • Retention pond on site appeared to be in good condition and functioning as designed.
  • The site appeared to be stable with greater than 70% vegetative cover. Some tire tracks from the landowner were observed on the stockpile to the eastern side of the pad but did
2814524 Primary Facility 2018-12-12 Routine/Complete Inspection No Violations Noted Well appears to be good condition and is producing. Well is currently being serviced.
2820824 Primary Facility 2018-12-27 Routine/Complete Inspection No Violations Noted On 12/27/2018 at approximately 11:00, I conducted an inspection of the George Godwin Well Site located in Cross Creek Township, Washington County. This inspection was in response to an incident which occurred on 12/23/2018. I met with Hannah McAvoy (Range Resources) on site to discuss the incident and associated remediation.

On 12/23/2018 at 4:10 AM, a hose in the well cellar was found to not be attached to a half round. Once the error was identified the pump was shut down. Operator determined that approximately 25 gallons of elevated conductivity water released to the graveled well pad. EAP Industries used a super vac to remove all of the impacted material which was placed into a roll-off box.

During an inspection of the project area, the following was observed:

  • The Act 9 sign located at the site entrance was in good condition.
  • Location where approximately 25 gallons of high conductivity water released appeared remediated.
  • Air rig in operation on site during inspection.
  • Production fluid/cond
2827648 Primary Facility 2019-01-16 Routine/Complete Inspection No Violations Noted At the time of inspection the the well site was covered with 2 inches of snow. the Act 9 site ID sign installed. PPC plan, ER plan, Waste Control and Disposal plan on site. Well drilled and equipped for production. Well tag present. Well apparatus, brine tank and GPU unit appeared to be sound condition. The Department recommends that the Operator continue to monitor the site weekly and after precipitation events.
2847539 Primary Facility 2019-03-04 Follow-up Inspection No Violations Noted Well shut in while Operator actively drills 125-28568.
