2663445 Primary Facility 2017-11-20 Incident- Response to Accident or Event No Violations Noted On 11/20/17 an INCDT inspection of the US Natural Resources well site was conducted accompanied by Range Resource's Dennis Furmanek in response to a spill reported as having occurred on 11/17/17. The following was observed:

>>The operator reported that on 11/17/17 an equipment failure in a GPU resulted in roughly 30 gallons of produced water was released to the pad.

>>The operator representative on site reported that the produced water infiltrated into the pad material and was found to have spread out roughly 2 feet below the pad surface. Impacted materials were excavated and test holes were dug to verify extent of spill and reported to show only trace impact. Slightly more than one box of materials were waiting to be removed from the site.

>>Standing water in both the excavated area and test holes was observed to be frozen at the time of inspection indicating negligible impact remaining at the time of inspection.

>>This spill constitutes a violation of 25 Pa. Code 78.57(a) in that the operator failed

2709714 Primary Facility 2018-03-22 Incident- Response to Accident or Event In Compliance with Policy On 3/22/18 an incident inspection was conducted at the US Natural Resources Well Site in response to a spill reported by the operator on 3/18/18 (CTS ID: 333123). The following was observed:

>>On 3/18/18 the operator reported that #9 GPU suffered an equipment failure resulting in a spill of roughly 10 gallons of produced water to the pad surface on 3/17/18. Cleanup was begun shortly after discovery by Specialized Professional Services Inc.

>>Excavation of impacted materials was completed the same day and one roll off box of materials was being held on site awaiting final disposal at the time of inspection.

>>An area of fresh gravel was observed on site of corresponding size to the reported spill volume.

>>This incident is being regarded as in compliance with policy.

Ten gallon spill from GPU. Occurred sat morning cleaned up sat afternoon. All removed materials placed in roll off box pending disposal.

2709804 Primary Facility 2018-03-21 Routine/Partial Inspection No Violations Noted On 3/20/18 an inspection of the US Natural Resources Well Site was attempted in relation to an operator reported spill (CTS ID: 333123), deep snow prevented site inspection due to inability to access the site.
