The coil tubing rig was on site and placed on secondary containment. No observation of accelerated erosion or sedimentation in relation to the well pad or access road.
2615760 Primary Facility 2017-07-17 Incident- Response to Accident or Event Violation(s) Noted Response to spill reported to the Department on 7/16/2017 at 8pm. The Operator indicated that approximately 10 barrels of concrete was lost under the secondary containment at 6pm. The Operator indicated that they were pumping concrete into the production casing and lost circulation. The Operator observed concrete on the edge of the well pad. Plugging activity was stopped. The Operators inspection revealed concrete was returning to the surface via the annular casing under secondary containment. The well permit was on site. ERP onsite. The PPC plan was on site. SDS on site. E& S plan was on site. Operator representatives onsite during inspection B.A Lambie, Ronnie Venghaus and Mark Manning. The Departments inspection confirmed that concrete was last lost to the well pad and appeared to be under the containment. Waters of the Commonwealth were not impacted. The cement solidified prior to leaving the perimeter controls. The Department observed two open top tanks. One of the tanks had less than 2 feet of freeboa
2616547 Primary Facility 2017-07-19 Follow-up Inspection In Compliance with Schedule Follow up inspection to violations issued in relation to a release reported on 7/17/2017. The Operator removed the secondary containment in the area of the release. The impacted area appeared to be remediated, the concrete removed was placed on containment awaiting disposal. The Operator indicated that confirmation samples were collected. The attached photo contains flags indicating the confirmation sample locations. The open top tanks located on site had more than 2 feet of freeboard.

The Operator was not actively working on the site during the inspection. Secondary containment removed and placed in two open top roll off boxes. The contents of an additional three roll off boxes also located on site could not be verified. Well plugged. The berm located on two sides of the well site appeared to have small erosion rills but no evidence that suggests the sedimentation is leaving the location or impacting waters of the Commonwealth. The Department recommends the Operator continue to monitor the site weekly and after significant precipitation events.
