2764761 Primary Facility 2018-08-17 Incident- Response to Accident or Event Violation(s) Noted Inspection in response to Operator report of a 25bbl brine water release resulting from the brine tank catching on fire due to a lightening strike. At the time of inspection, Operator (Hannah McAvoy), cleanup contractor and Department oil and gas inspector (Paul Wyrwas) on site. Spill clean up in progress. Three trenches successively down-gradient of well site installed to catch spill and residual runoff from precipitation events. Excavated impacted dirt and gravel placed on plastic tarp pending disposal. Operator estimates 25 bbls of brine was present in the 50 bbl polytank at the time of the fire. Operator estimates that 5 bbls may have hit the ground the remainder was burned in the fire. Farm pond is located approximately 500ft southeast and down-gradient of well site. At the time of inspection no visual evidence of spill impacting this pond, as indicated by the presence of a sheen and or dead organisms was observed. At the time of inspection no E&S concerns were observed.

Department recommends Operator

2764886 Primary Facility 2018-08-17 Routine/Complete Inspection Pending On August 16, 2018 at 18:30 hours, a lightening strike occurred at well head hitting the brine tank and associated piping. Brine tank was grounded with copper wire connected to grounding rod. Afterwards, fire ignited the 100 bbl poly tank. Resident across Kooser street called landowner and local fire department. Landowner called Range production manager Doug Bargerstock. Doug arrived at site at 18:45 hours. Refer to photo 1. Doug was able to shut well in at wellhead eliminating gas supply to lateral lines. Fire company was able to suppress fire with water to extinguish fire around 19:15 hours. As precaution, Range dug three interceptor trenches along hill side to prevent any brine water to release from site and enter pond located at bottom of hill. Refer to photo 2. The fire burnt brine tank which contained at the time of fire approximately 20 to 30 bbl of brine water. On august 17,2018 at 08:30 hours, DEP OGI Paul Wyrwas performed a site inspection. Range contracted Drew Moving Service to perform

2770784 Primary Facility 2018-09-05 Follow-up Inspection Outstanding Violations - Viols Req'd Follow-up inspection to Operator reported spill on 8/16/2018. At the time inspection excavation associated with 8/16/2018 incident appears to be completed. Excavated area of impact has been filled and seeded and mulched. The three trenches successively down-gradient of well site installed to catch spill and residual runoff from precipitation events previously installed still in place pending finalization of cleanup and site restoration. E&S controls installed. No E&S issues.

Department recommends Operator continue to perform routine maintenance and monitoring of site pending final restoration and cleanup approval from Department.
