2841668 Primary Facility 2019-02-19 Administrative/File Review In Compliance with Policy ADMIN inspection conducted in response to an operator reported release of approximately 20 gal of brine.

Department contacted operator rep Eric Stahl by phone. Mr. Stahl states a release of approx. 20 gal of brine occurred on 2-18-19. Mr. Stahl states the brine tank valve nipple failed and dripped brine onto the ground. Mr. Stahl does not feel waterways are affected. Mr. Stahl also states the brine tank has been emptied and the valve will be replaced and clean-up will begin soon. Operator appears to be in compliance at this time.

2842880 Primary Facility 2019-02-19 Incident- Response to Accident or Event Violation(s) Noted FUI to inspection record # 2841668 where an operator reported a release of approximately 20 gal of brine.

At time of inspection well appears equipped for production. Brine tank located in an earthen dike. According to operator rep Eric Stahl, brine tank was emptied of fluid and the faulty nipple will be replaced (see above inspection record). At this time the ground under the brine tank valve appears wet (photo #1). No active dripping/leaking observed. No fluid is observed in the earthen dike. At this time Department unable to determine if the nipple has been replaced. Violations are being issued for discharge of brine or other fluids to the ground and for disposal of waste inconsistent with the rules and regulations of DEP. A follow up inspection will be conducted at a later date.

2851272 Primary Facility 2019-03-08 Administrative/File Review In Compliance with Policy ADMIN inspection to inspection record #2842880. Department received operator response to NOV (photo #1).

Violations issued on 2-19-19 remain outstanding. A follow up inspection will be conducted at a later date. - Invalid Complaint Id. - 33

2853898 Primary Facility 2019-03-14 Compliance Evaluation No Violations Noted Compliance Evaluation Inspection to inspection record #2842880 where violations were issued for discharge of brine or other fluids and for disposal of waste inconsistent with DEP rules and regulations.

At time of inspection it appears impacted soil has been excavated and placed in a plastic container. This container is located near the brine tank in the earthen dike and is covered with metal sheeting (photo #1). Excavated area covered with wooden structure (photo #2). Department recommends operator dispose of impacted soil in a manner permitted by law and provide Department with disposal manifests when available. Operator appears to be in compliance at this time. Violations issued on 2-19-19 have been corrected.
