2789277 Primary Facility 2018-10-16 Incident- Response to Accident or Event Violation(s) Noted Inspection in response Operator report of a brine spill. Spill was discovered by Operator on 10/15 /2018 @ 15:30 and called into Department the same day at 17:05.

On 10/16/2018 WQS Santiapillai spoke with Operator (Eric Staul) who stated that the cause of the spill was a faulty discharge valve on the brine. Operator estimates 45gallons to have been released. Operator stated that the brine tank was pumped out and faulty valve replaced. Cleanup pending.

At the time of inspection well appears equipped for production. No active leaks observed from brine tank discharge valve and valve appears to have been repaired. Area of impact from reported release appears to be localized and contained to well site. Cleanup has not commenced at the time of this inspection.

Department requests Operator to perform cleanup in accordance with Spill Policy. Department requests Operator provide a written response detailing the cause of the spill, type and quantity of material released, impacted materials, and remedial actions

2834379 Primary Facility 2019-01-28 Compliance Evaluation No Violations Noted Compliance Evaluation Inspection to inspection record #2789277 where a violation was issued for operator discharge of brine or other fluids on or into the ground.

At time of inspection well site snow-covered. An operator-reported release of brine occurred on 10-15-18. Area of release appears mulched. Hay bales installed on NW side of well site, down-gradient of release area (photo #1). No E and S issues observed. Department contacted operator rep Carla Henry by phone from site. Ms. Henry states the site was backfilled, seeded, and mulched on 12-11-18. Ms. Henry also states disposal manifests will be provided to the Department today. Department received disposal manifests at 10:07. Operator appears to be in compliance at this time. Violation issued on 10-16-18 has been corrected.
