Well site has approximately 95% vegetative cover. Large ruts have formed in the access road. As a result sediment is carried down the access road and beyond the well site. The department recommends the following: 1. The access road should be graded. 2. A clean rock large enough to stabilze the road should be put down to give a good base. 3. A smaller rock should then be compacted in over the subbase. 4. Lastly a waterbar with a rock outlet.should be implimented halfway down the steepest slope of the road as it decends to the well. Operator should continue to monitor the well site for erosion and sedimentation.

Well site has approximately 95% vegetative cover. Large ruts have formed in the access road. As a result sediment is carried down the access road and beyond the well site. The department recommends the following: 1. Grade the access road in the areas that have developed large ruts. 2. Apply large clean rock to low lying areas and section of access road that has a down slope gradient. 4. A waterbar with a rock outlet.should be implimented halfway down the steepest slope of the road as it decends to the well. 4. Lastly the well pad should be rocked to stabilize and prevent erosion and sedimentation. Operator should continue to monitor the well site for erosion and sedimentation.
2462287 Primary Facility 2016-03-01 Complaint Inspection Violation(s) Noted Inspection performed in response to a complaint of an ongoing leak received by the Department on 2/29/2016. Access road gated and locked. At the time of inspection well appears equipped for production. Well tag present and affixed in a permanent manner. Brine tank located on site and appears full. During inspection, Department observed two active leaking points. First active leak observed at the T-joint connector of the inflow flange located at the top of the brine tank. An approximate area measuring 4’x4’ directly below the leak appears to be impacted, as indicated by visible staining of the ground/vegetation. Second active leak observed at the outflow flange located at the base of the brine tank. An approximate area measuring 2’x2’ directly below the leaking outflow flange appears to be impacted, as indicated by visible dampness and pooling of fluids, with no evidence of sheen. Production chart located onsite is dated 8-20-15.
