During the inspection, Department observed an active leak from the base of the brine tank, as indicated by a slow steady drip. An approximate area measuring 2’x2’ directly below the leak appears to be impacted, as indicated by visible dampness of the gravel. Release appears to be localized to within the well site. Production chart located on site and dated 7-1-2016.

Department’s attempts to contact Operator regarding above concerns were unsuccessful due to an invalid contact number.

Department recommends Operator perform cleanup in accordance with Spill Policy and follow-up with Department upon investigation of reported concerns and to quantify release and identify all materials released if applicable within 24 hours of receipt of this inspection.

Violations are being issued under 25 PA CODE 78.57. Additional violations may be forthcoming pending Departmental investigation. Department will
