1770823 Primary Facility 2008-12-30 Routine/Complete Inspection Outstanding Violations - No Viols Req'd rosion control measures were undergoing repairs while onsite at the time of the inspection. Noted very little erosion and sedimentation on the access road and well pad, and noted there appeared to have been no off-site transport of sediment. A contractor onsite was repairing an area to the left of the access road going down the hill that had been impacted by heavy volumes of runoff and had slid. Noted an area along the filter fabric fence that needed to be adressed and reinforced with hay bales. Notified the contractor onsite of this problem and spoke with operator representative Eric Findlay later and informed him of this as well as the general condition of the site and access road.

Brine tank onsite and appeared to be in sound condition. Noted that the BMP's implented along the access road appeared to be functioning properly and minimizing accelerated erosion & sedimentation. Noted that the operator had mulched the disturbed site areas with hay/straw. The operator should continue to montior the site & access road for accelerated erosion & sedimentation until final restoration activities are completed.

WELL HAS 60LBS ON 1.5" AND 145LBS ON 4.5" ERIC SAID THAT WHEN THEY FIRST ROCKED UP THE WELL THEY IMMEDIATLEY VENTED AFTER DISCOVERING IT HAD 920LBS ROCK PRESS. THEY HAD CREWS WORK ROUND THE CLOCK SAT AND SUN AND HAD THE WELL TURNED INTO THE LINE IN 2 DAYS. E&S CONTROLS WERE IN PLACE, RESTORATION WAS LOOKING GOOD, EVERYTHING LOOKED GOOD AT THE SITE
2362711 Primary Facility 2015-04-20 Incident- Response to Accident or Event No Violations Noted Inspection in response to the a spill reported to the Department on 4/17/2015. The Operator reported a <1 gallon brine spill due to a possible bullet fired from a nearby shooting range. At the time of inspection the Department observed what appeared to be several bullet marks on the well apparatus and a small patch on the brine tank with stains running down the side of the brine tank. No brine appears observed on the ground and no water ways affected. The Department suggests talking to the range officials about the potential dangers associated with the type of behavior and installing measures to prevent this type of situation in the future.
