Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1734497 Primary Facility 2008-09-05 Routine/Complete Inspection No Violations Noted Drilling for production hole started on 08/13/08, and the access hole on 08/15/08. Lateral holes started and finished between 08/25/08 and 09/04/08. No accelerated E&S onsite. E&S plan implemented properly. Pit freeboard ok.
1736909 Primary Facility 2008-08-19 Drilling/Alteration No Violations Noted Coal bed methane well rig @ 1011' ready to start drilling curve @ inspection. Pit/freeboard/E&S Ok.
1762773 Primary Facility 2009-01-06 Complaint Inspection No Violations Noted Inspection performed as a result of a report of runoff from the site impacting neighboring properties. Drilling had been completed at the time of the inspection, and the site had been regraded and reclaimed. Found no accelerated erosion & sedimentation on the access road or the site. Noted that the vegetative cover appeared to meet the 70% uniform coverage criteria at the time of the inspection. Appeared that all erosion control measures implemented had performed properly and that there was no evidence of off-site transport of sediment via run-off. The operator should continue to monitor the site and access road to insure that the erosion control meausres continue to function properly in preventing the off-site transport of sediment via runoff.
1773497 Primary Facility 2009-02-13 Routine/Complete Inspection No Violations Noted diameter of standpipe increased to reduce noise from 2" to 4",
1773685 Primary Facility 2009-01-30 Routine/Complete Inspection No Violations Noted leak around 7" fixed, vent stack installed for noise

1843600 Primary Facility 2009-11-12 Site Restoration No Violations Noted Well is drilled and does not appear to be in production at the time of inspection. There is a wet area around the pump jack and propane tanks. That area should be filled in to minimize erosion and sedimentation. Brine tank is empty and in sound condition. There is adequate vegetation present. The bank along the access road ditch should be reseeded and mulched. The well site is restored.
2314905 Primary Facility 2014-10-22 Routine/Complete Inspection No Violations Noted At the time of inspection the well appeared to be drilled and equipped, however the well is temporarily inactive. The Department suggests the Operator perform routine maintenance to the beginning of the access road in the area of the first level spreader. The road in that area has collapsed and is making it difficult to navigate for inspections.
