2509274 Primary Facility 2016-08-09 Routine/Complete Inspection No Violations Noted At the time of inspection well appears equipped for production. Well tag present and affixed in a permanent manner. Two brine tanks located on site, within earthen dike. At the time of inspection brine tanks appears to structurally compromised due to rusting as indicated by the presence of holes, located on the upper half of tank. Holes varying in size with the largest approximately the size of a quarter. No active leaking observed. Department called left message with answering service requesting Operator contact Department in regards to above findings and to recommend Operator correct this issue in order to prevent a potential pollution event. Site stable with no E&S issues in relation to site and/or access road. Department will conduct a follow-up inspection.
2509277 Primary Facility 2016-08-10 Administrative/File Review No Violations Noted On 8/10/16 @ 10:00am Department called Operator and left a message requesting Operator to call Department in regards to 8/9/16 inspection.

2517537 Primary Facility 2016-09-07 Routine/Partial Inspection No Violations Noted Inspection performed as a follow-up to inspection ID 2509274 dated 8/9/16. Department met with Operator on site to address structural integrity concerns of the brine tanks. Operator stated that all production fluids will be plumbed into tank 5H that is structurally sound and the tank will be repainted. Tank 5H will be the primary tank for production fluid collection. Other three tanks that are structurally compromised due to rusting will be drained of any remaining fluids and remain plumbed.
2705988 Primary Facility 2018-03-15 Routine/Complete Inspection No Violations Noted At the time of inspection well equipped for production. Four brine tanks within earthen dike. Three of the four tanks showing signs of rusting and appear to be structurally compromised as indicated by the presence of holes on the upper half of tanks. No active and/or evidence of old leaks/spills observed. As indicated by Operator (see inspection record 2517537) the three compromised tanks are not in use. No E&S issues.
