1874971 Primary Facility 2010-04-06 Complaint Inspection No Violations Noted The Department conducted an investigation in relation to a complaint. The complainant stated that high amounts of dust were being blown on to his property. The Department contacted the operator and noted that the well was in the process of being fractured.

1884909 Primary Facility 2010-05-14 Routine/Complete Inspection No Violations Noted The operator submitted a request for a six month site restoration extension on 4/7/10 due to pipeline and frac equipment scheduling difficulties. The well reached total depth on 8/27/09 and the ID sign and E&S plan was on site. All E&S controls were installed properly and need to be maintained and inspected after every significant rainfall event. The following extension has been approved, and the deadline for the site restoration is 11/27/10. The Department requests that the fill slop be inspected on a regular basis to hinder any accelerated erosion along with weekly inspections and inspections that follow any significant precipitation that might cause a runoff event.
2132710 Primary Facility 2013-01-23 Incident- Response to Accident or Event No Violations Noted On 1/22/13 Atlas called in a small 3 gallon diesel fuel spill on the the well pad. Inspected site. Spill was cleaned up with no visible staining observed.
2395129 Primary Facility 2015-08-03 Site Restoration No Violations Noted Inspection in response to the Operators submission of a "Well Site Restoration Report on 7/22/2015. At the time of inspection earth disturbance activities at the site authorized by the Well Permit are complete, the site has been stabilized and the Post Construction Stormwater Management BMP's have been installed.
