1843344 Primary Facility 2009-11-02 Routine/Complete Inspection Violation(s) Noted Inspection performed to evaluate the well site after a 790+/- gallon diesel fuel spill occurred on 10/30/09. The diesel fuel release reportedly was the result of an improperly connected fuel line on Union Drilling's rig. It was estimated that approximately 200-250 gallons were recovered from the time of the spill until 11/2/09. The operator contained the spill on-site in a diversion ditch, therefore the waters of the commonwealth were not directly affected. The operator used booms and absorbant pads to collect as much of the liquid fraction as possible. The used booms and pads were then placed in 55 gallon drums for containment. Dennis Stivanson was on-site 10/30/09 and observed clean up efforts made by the operator, union drilling, and an environmental cleanup company.
1874649 Primary Facility 2010-03-31 Compliance Evaluation No Violations Noted The Department conducted a compliance evaluation inspection in response to the violations noted on 11/2/09. Specifically, there was 790 gallons of diesel fuel discharged to the ground. The ground was excavated and all contaminated soil was taken off site and disposed of properly. The Department noted that all of the violations have been abated.

2250424 Primary Facility 2014-03-04 Routine/Complete Inspection No Violations Noted At the time of inspection the well appeared to be drilled and equipped for production. The brine tanks and well apparatus appear to be in sound condition. No observation of accelerated erosion or sedimentation and the site appears to be restored and meets the 70% uniform vegetative coverage.
2613237 Primary Facility 2017-07-10 Routine/Complete Inspection No Violations Noted Well drilled and equipped for production. Well tag present. Well apparatus and brine tank appear to be in sound condition. No accelerated erosion or sedimentation observed on well site or access road.
