Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1861333 Primary Facility 2010-01-15 Routine/Complete Inspection No Violations Noted On January 15, 2010 I conducted an inspection for an OG 57 Request for waiver of distance requirement. Also on site was DEP WQS Nick Staffieri. The well site is proposed to be constructed within 100 ft. from an unnamed tributary to Buffalo Run. Well was site staked. Construction has not begun at site. Proposed E & S plan should protect stream from adverse impact during site/well development if properly implemented & maintained. Drill cuttings are to be land farmed on site. Operator must ensure that the surface of the land farm area is at least 20 inches above the seasonal high groundwater table.
1868774 Primary Facility 2010-03-08 Routine/Complete Inspection No Violations Noted Well is being drilled currently.
1875903 Primary Facility 2010-04-06 Routine/Complete Inspection No Violations Noted drilling complete, not equipped for production, rough graded.
1899779 Primary Facility 2010-07-06 Routine/Complete Inspection No Violations Noted Site inspection for GP056510810 and GP076510801 for a pipeling project. Met Mike Morgart on sit to look at any possible wetland impacts due to hydric soils being in the project area. The small drainage has been altered at some point and the land use is a mow field so other than a few riparian vegeation species there are no other wetland indicators that we observed that would be impacted by this project.
2349164 Primary Facility 2015-03-04 Routine/Complete Inspection No Violations Noted Tag, tank, e&s ok.
