2077870 Primary Facility 2012-06-14 Incident- Response to Accident or Event Violation(s) Noted Met onsite with operator representative Jarrod Shultz regarding the discharge of approximately 290 gallons of frac water (pre-stimulation). The water was an approximate mix of 90% freshwater and 10% reuse water. The spill resulted due to valves left open on the filtration system. The subsequent release was iniaitally contained on plastic, however due to improper construction the fluid did not flow back into the frac impoundment and was released onto the ground. Upon arrival onsite Chevron had ceased operations and was beginning to move the filtration equipment to access the area affected by the discharge. Excavation crew will be onsite as of the day of the inspection and upon completion of excavation of the affected media ECS&R had been contracted to perform confirmatory samples to insure that all affected media had been excavated. Operator incident report is attached to the inspection.

After reviewing the operator submitted documentation it has been determined that the operator effectively remediated the discharge area and had removed all of the affected media.
